流行性腮腺炎累及中枢神经系统患儿脑干听觉诱发电位的观察与分析

目的 分析并探讨流行性腮腺炎累及中枢神经系统 (CNS)患儿脑干听觉诱发电位 (BAEP)的变化规律和应用价值。方法 对 473例流行性腮腺炎累及CNS患儿按脑脊液和脑电图的不同变化分三组 ,应用肌电 /诱发电位仪进行BAEP描记分析。结果  473例BAEP检测异常 78例 ,总异常率为 16 5 % (78/ 473) ,其中脑电图异常组BAEP异常 14例 ,异常率 14 7% (14/ 95 ) ,脑脊液异常组BAEP异常 2 5例 ,异常率 17 4% (2 5 / 144 ) ,脑电图与脑脊液均异常组BAEP异常 39例 ,异常率 16 7% (39/ 2 34) ,三组之间相互比较 ,均为P >0 0 5 ,无显著性差异。BAEP各波异常中Ⅰ波PL延长异常累计 15例次 (3 17% ) ,其余各波PL延长、IPL延长累计共 73例次 (15 43% ) ,两者比较P <0 0 1差异有极显著性。 473例患儿有耳聋 4例 (0 85 % )。结论 流行性腮腺炎累及CNS出现BAEP异常多见 ,但耳聋不多 ,其BAEP异常率与脑电图及脑脊液改变的关系不大 ,脑干水平听觉传导通路的损害发生率高于耳蜗和听神经 ,但为可逆性损害 ,耳蜗和听神经损害可导致耳聋常为不可逆性 ,流行性腮腺炎累及CNS时应做BAEP筛查 Objective To analyze and discuss the regularity and application value of brainstem auditory evoked potential (BAEP) in mumps patients with central nervous system (CNS). Methods 473 cases of mumps-affected children with CNS were divided into three groups according to different changes of cerebrospinal fluid and electroencephalogram. BAEP was performed by EMG / EPG. Results A total of 473 cases of abnormal BAEP were detected in 78 cases, with a total abnormality rate of 16 5% (78/473). There were 14 abnormal BAEP and 14 7% (14/95) EEG abnormalities, and abnormal BAEP 2 (N = 5), the abnormality rate was 17.4% (25/144). There were 39 cases of abnormal BAEP and abnormal rate of 16.7% (39/234) in EEG and cerebrospinal fluid. The three groups were all P > 0 0 5, no significant difference. In all BAEP wave anomalies, 15 cases (33.17%) were abnormal abnormalities in PL wave, and all other PLs were prolonged and 73 cases were IPL prolongation (15 43%). There was a significant difference between P <0 01 Significance. 473 children had deafness in 4 cases (0 85%). CONCLUSIONS: Mumps can affect the appearance of BAEP abnormalities in the CNS. However, there is not much deafness in the mumps. The abnormal rate of BAEP is not related to the change of EEG and cerebrospinal fluid. The damage of the auditory nerve conduction pathway at the brainstem level is higher than that of the cochlear and auditory nerve. However, reversible damage, cochlear and auditory nerve damage can lead to deafness often irreversible, mumps should be done when the involvement of CNS BAEP screening
