目的 探讨p16、p5 3和CD4 4V6在胃癌发生、发展及淋巴结转移的作用机理和意义。方法 用免疫组化ABC法检测p16、p5 3和CD4 4V6在 93例胃癌组织中的表达。结果 93例胃癌中p16、p5 3和CD4 4V6的阳性表达率分别为 39 8%、5 0 5 %、41 9%。p16蛋白在淋巴结转移阳性组的阳性表达率 30 8% ,显著低于淋巴结转移阴性组的阳性表达率 6 0 7% (P <0 .0 1)。p5 3在进展期胃癌、淋巴结转移阳性组的阳性表达率 5 4 9%、5 8 5 % ,显著高于早期胃癌、淋巴结转移阴性组的阳性表达率 18 2 %、32 1% (P <0 .0 5 ,P <0 .0 5 )。CD4 4V6在进展期胃癌、淋巴结转移阳性组的阳性表达率 43 .8%、5 2 .3 % ,显著高于早期胃癌、淋巴结转移阴性组的阳性表达率 9 1%、17 9% (P <0 .0 5 ,P <0 .0 1)。结论 p16蛋白的缺失表达对胃癌的淋巴结转移起重要作用。p5 3、CD4 4V6的高表达对胃癌的浸润、转移起重要作用 ,对分析胃癌的生物学行为和判断预后有重要意义。
Objective To investigate the mechanism and significance of p16, p53 and CD4 4V6 in the occurrence, development and lymph node metastasis of gastric cancer. Methods The expression of p16, p53 and CD4 4V6 in 93 cases of gastric cancer was detected by immunohistochemical ABC method. Results The positive expression rates of p16, p53 and CD4 4V6 in 93 cases of gastric cancer were 39 8%, 50 5% and 41 9%, respectively. The positive expression rate of p16 protein in the lymph node metastasis positive group was 30 8%, which was significantly lower than that of the lymph node metastasis negative group (60.7%) (P < 0.01). The positive expression rate of p53 in advanced gastric cancer and lymph node metastasis was 54.9% and 58.5%, which was significantly higher than that of early gastric cancer and lymph node metastasis. The positive expression rate of p53 was 18 2%, 32 1% (P <0. .0 5, P <0 .0 5 ). The positive expression rate of CD4 4V6 in advanced gastric cancer and lymph node metastasis group was 43.8%, 52.3%, which was significantly higher than that of early gastric cancer and lymph node metastasis negative group (9 1%, 17 9%) (P < 0 .0 5 ,P <0 .0 1). Conclusion The deletion of p16 protein plays an important role in lymph node metastasis of gastric cancer. The high expression of p53 and CD4 4V6 plays an important role in the invasion and metastasis of gastric cancer. It is of great significance to analyze the biological behavior of gastric cancer and determine the prognosis.
