目的探讨应用兔肢体软组织肿瘤模型实施术前大分割容积弧形调强放疗(volumetric modulated arc radiotherapy,VMAT)的可行性。方法成功构建30只兔VX2软组织肿瘤模型,2周后随机分为VMAT大分割放疗组10只(VMAT组)、常规分割调强放疗(intensity modulation radiated therapy,IMRT)放疗组10只(IMRT组)和对照组10只,给予的单次剂量分别为3Gy、2Gy和不采取任何处理。放疗结束后,3组行MRI扫描评估,并采用Envision法检测肿瘤组织Ki-67表达率。结果 VMAT组和IMRT组肿瘤体积[(21.11±2.36)、(23.56±3.21)cm3]和肿瘤组织Ki-67表达率[(49.67±2.34)%、(51.00±2.39)%]均小于对照组[(46.78±3.38)cm3,(68.22±5.21)%](P<0.01),VMAT组与IMRT组比较差异无统计学意义(P>0.05)。结论大分割VMAT在兔肢体恶性软组织肿瘤的术前放疗中可取得与常规分割IMRT相同疗效。
Objective To investigate the feasibility of preoperative large volume volume modulated modulated radiotherapy (VMAT) using rabbit limb soft tissue tumor model. Methods Thirty rabbits with VX2 soft tissue tumor were successfully established. Two weeks later, they were randomly divided into VMAT group (VMAT group), IMRT group (IMRT group), conventional IMRT group And control group of 10, given a single dose of 3Gy, 2Gy and do not take any treatment. After the end of radiotherapy, three groups were evaluated by MRI scan, and the expression of Ki-67 in tumor tissue was detected by Envision method. Results The tumor volume in VMAT group and IMRT group was significantly lower than that in control group [(21.11 ± 2.36), (23.56 ± 3.21) cm3] and Ki-67 in tumor tissues [(49.67 ± 2.34)%, (51.00 ± 2.39)%] (46.78 ± 3.38 cm 3, 68.22 ± 5.21%, P <0.01). There was no significant difference between VMAT group and IMRT group (P> 0.05). Conclusions Large segmental VMAT can achieve the same effect as conventional segmental IMRT in the preoperative radiotherapy of rabbit limb malignant soft tissue tumors.
