目的 探讨妊娠母亲和新生儿的血清胆红素与肝酶的相互关系及意义。方法 同时测定118组配对妊娠母亲、脐血、新生儿和42例对照组成人的血清总胆红素(BilT)、结合胆红素(BilD)和肝酶(ALT、AST、GGT、LDL),并将母亲和新生儿的血清BilT、BilD与肝酶结果作相关分析。结果 ①血清BilT、BilD含量高低依次为新生儿>脐血>妊娠母亲、对照组(P<005);妊娠母亲与对照组的血清BilT、BilD差异无显著意义(P>005)。②血清ALT、AST、LD—L含量高低为母亲>对照组(P<005);GGT含量高低为新生儿、脐血>母亲、对照组(P<005),新生儿与脐血之间、母亲与对照组之间GGT含量差异无显著意义(P>005);血清AST、LDL含量高低为新生儿>母亲、对照组(P<005),新生儿与对照组的血清ALT差异无显著意义(P>005)。③母亲的血清胆红素水平与肝酶含量相关(P<001),新生儿的血清胆红素含量与肝酶无相关关系(P>005)。结论 妊娠母亲与对照组成人的血清胆红素含量无统计学差异,新生儿出生后血清胆红素水平逐渐增高。母亲的血清BilT、BilD含量与肝酶的变化一致,新生儿的血清BilT、BilD的变化不受肝酶含量的影响。
Objective To investigate the relationship between serum bilirubin and liver enzymes in pregnant women and newborns and its significance. Methods Serum total bilirubin (BilT), bilirubin (BilD) and liver enzymes (ALT, AST, GGT, LDL) in 118 matched pregnant mothers, cord blood, newborns and 42 controls were measured simultaneously. The correlation between serum BilT, BilD and liver enzymes in mothers and newborns was analyzed. Results ① The levels of BilT and BilD in serum were in the order of newborn> cord blood> pregnant mothers and control group (P <0.05). There was no significant difference in serum BilT and BilD between pregnant mothers and control group (P> 0.05). ② Serum levels of ALT, AST and LD-L were significantly higher in mothers than in controls (P <0.05); levels of GGT in neonates and umbilical cord blood> mothers and controls (P <0.05) There was no significant difference in GGT content between mothers and controls (P> 0.05). Serum levels of AST and LDL were no significant difference between neonates and mothers (P <0.05) (P> 005). ③ The level of serum bilirubin in the mother was correlated with the level of liver enzymes (P <001). The serum bilirubin level in the newborns was not related to the level of liver enzymes (P> 005). Conclusion The serum bilirubin levels in pregnant mothers and control subjects are not statistically different, and the serum bilirubin levels of neonates after birth are gradually increased. Mothers serum BilT, BilD content consistent with changes in liver enzymes, neonatal serum BilT, BilD changes from the liver enzyme content.
