目的探讨粘连性小瞳孔白内障超声乳化吸除的技术。方法我们对35例45眼粘连性小瞳孔白内障患者,采用撕除虹膜面机化膜、或用玻璃体切割头咬除瞳孔缘机化膜等方法使瞳孔扩大至4~5mm,并在囊袋内完成白内障超声乳化吸除并推注法植入人工晶体。结果术后1个月裸眼或最佳矫正视力:≥1.0者7眼(15.6%),0.5~0.8者25眼(55.6%),0.1~0.4者11眼(24.4%),0.06~0.08者2眼(4.4%)。术后随访瞳孔情况:瞳孔形状呈圆形或近圆形,均有瞳孔对光反射。结论采用撕除虹膜面机化膜、或用玻璃体切割头咬除瞳孔缘机化膜等方法能使瞳孔扩大至4~5mm,既能顺利完成白内障超声乳化手术,术后又能保留瞳孔的功能。
Objective To study the technique of phacoemulsification for small pupil cataract. Methods We retrospectively analyzed the results of 35 patients with 45 eyes with small pupil cataract who were treated with ripper iris facial mask or with vitrectomy head to dislodge the pupil marginalization machine to make the pupil expand to 4 ~ 5mm, Cataract phacoemulsification and implanting intraocular lens implantation were completed. Results One month after operation, the best corrected visual acuity was achieved in 7 eyes (15.6%) ≥1.0, 25 eyes (55.6%) in 0.5 ~ 0.8 eyes, 11 eyes (24.4%) in 0.1 ~ 0.4 eyes, Eye (4.4%). Follow-up pupil conditions: pupil shape was round or nearly circular, both pupil light reflex. Conclusions The method of removing the iris faceted machine membrane or using the vitrectomy head to bite out the pupil margin machine membrane can enlarge the pupil to 4 ~ 5mm, which can not only successfully complete the phacoemulsification of the cataract but also retain the pupil function after the operation .
