目的归纳高位食管癌颈胸腹三切口根治术临床诊疗。方法对230例高位食管癌病患行颈胸腹三切口食管癌根治术。结果全组围手术期无死亡。16例产生颈部吻合口漏,通过引流换药等疗法痊愈;无胸部并发症。10例术后5个月至1年死于远处转移并发症,1年生存率92%,3年生存率40.0%,存活最长者近10年。结论三切口疗法对于根治性切除高位食管癌安全有效。 Objective To summarize the clinical diagnosis and treatment of high incidence esophageal neoplasms with cervical and thoracic incision. Methods 230 cases of esophageal cancer patients underwent cervical thoracoabdominal triple incision esophageal cancer radical mastectomy. Results The whole group had no death during perioperative period. 16 cases of cervical anastomotic leakage, cure by drainage and dressing therapy; no chest complications. Ten patients died of distant metastasis from 5 months to 1 year after operation. The 1-year survival rate was 92%, the 3-year survival rate was 40.0%, and the longest survival rate was nearly 10 years. Conclusion Three incisions are safe and effective for radical resection of esophageal cancer.
