目的探讨胸部计算机X线摄影(CR)检查、直接数字平板X线成像系统(DR)检查、16排螺旋CT常规剂量及16排螺旋CT低剂量扫描技术在健康人群胸部体检中的临床诊断价值。方法选择本院体检中心2013年8月—2014年10月随机申请健康体检人群行胸部检查者872名,其中男453名,女419名;年龄28~60岁,平均(42.1±6.7)岁。219名行胸部CR检查,221名行胸部DR检查,227名行16排螺旋CT常规剂量扫描,205名行16排螺旋CT低剂量扫描。结果胸部DR检查的阳性诊断率为15.8%,明显优于CR组的9.1%,差异有统计学意义(P<0.05);16排螺旋CT常规剂量扫描阳性诊断率为26.9%,低剂量扫描阳性诊断率为22.0%,差异无统计学意义(P>0.05));16排螺旋CT常规剂量扫描的阳性诊断率明显优于DR组(P<0.05)。结论常规CT检查胸部疾病诊断阳性率明显优于X线胸片检查,16排螺旋CT低剂量扫描可以在健康体检人群中广泛应用。
Objective To investigate the clinical value of chest computed tomography (CR) examination, direct digital plain radiography (DR) examination, conventional 16-slice spiral CT and 16-slice spiral CT low-dose scanning in the chest examination of healthy subjects. Methods The physical examination center of our hospital was selected from August 2013 to October 2014. 872 chest exams were randomly selected for the health examination population, including 453 males and 419 females; aged 28-60 years (mean, 42.1 ± 6.7) years old. 219 chest X-ray examination, 221 chest thoracic DR examination, 227 conventional 16-slice spiral CT scan and 205 low-dose 16-slice spiral CT scan. Results The positive diagnostic rate of chest DR was 15.8%, which was significantly better than that of CR (9.1%), the difference was statistically significant (P <0.05). The positive diagnostic rate of conventional 16-slice spiral CT scan was 26.9% The diagnostic rate was 22.0%, the difference was not statistically significant (P> 0.05). The positive diagnostic rate of 16-slice spiral CT scan was significantly higher than that of DR group (P <0.05). Conclusion The positive rate of CT diagnosis of chest diseases is significantly better than that of X-ray. Low-dose 16-slice spiral CT scan can be widely used in the health examination population.
