[目的]观察祛瘀滋阴汤联合强的松治疗系统性红斑狼疮疗效。[方法]使用随机平行对照方法,将100例住院患者按随机数字表法分为两组。对照组50例强的松(上海信谊药厂有限公司,国药准字H31020771),重度期,≥1mg/(kg.d)+标准环磷酰胺冲击疗法(5mg/m2体表面积,1次/4周);轻中度期,5mg/(kg.d);稳定期,<15mg/d;红斑明显,抗疟药;关节痛明显,非甾体抗炎药,2次/d,口服。治疗组50例清热养阴,补益肝肾,方选祛瘀滋阴汤,玄参15g,生地、麦冬、知母、黄柏各10g,金银花12g,地骨皮、青蒿各9g,生甘草6g。肝肾不足,熟地、山茱萸、菟丝子、枸杞子、女贞子、墨旱莲;面色萎黄,头晕眼花,苔薄舌淡,脉弱,加当归、白芍、菊花、地龙;手足冰凉,苔薄白,脉沉细,加黄芪、赤芍、红花、桂枝、鸡血藤、去知母、黄柏、金银花、地骨皮;油腻性脱屑,加生山楂,侧柏叶、土太黄、车前子,水煎服,2剂/d,早晚口服。西药治疗同对照组。连续治疗90d为1疗程。观测临床症状、疾病缓解时间、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组治愈20例,显效10例,有效5例,无效15例,总有效率68.00%。对照组治愈15例,显效15例,有效2例,无效18例,总有效率52.00%。治疗组疗效优于对照组(P<0.05)。疾病缓解所需时间比较治疗组优于对照组(P<0.05)。[结论]祛瘀滋阴汤联合强的松治疗系统性红斑狼疮疗效显著,值得推广。
[Objective] To observe the curative effect of Quyu Ziyin Decoction combined with prednisone on systemic lupus erythematosus. Liver and kidney deficiency, Rehmannia, dogwood, dodder, medlar, Ligustrum lucidum, Eclipta; pale complexion, dizziness, moss thin pale, pulse weak, plus angelica, white peony root, chrysanthemum, earthworm; hand and foot cold, moss Thin white, thin pulse, plus Astragalus, red peony root, safflower, cassia twig, Millettia to Anemarrhena, cork, honeysuckle, Digupi; greasy desquamation, add Hawthorn, Oriental Arborvitae, soil too Yellow, Plantago, Shuijianbi, 2 / d, morning and evening oral. Western medicine treatment with the control group. Continuous treatment of 90d for a course of treatment. Observed clinical symptoms, disease remission time, adverse reactions. Continuous treatment of 2 courses to determine the efficacy. [Result] The treatment group cured 20 cases, markedly effective in 10 cases, effective in 5 cases, invalid in 15 cases, the total effective rate was 68.00%. The control group was cured in 15 cases, markedly effective in 15 cases, effective in 2 cases, ineffective in 18 cases, the total effective rate was 52.00%. The treatment group was better than the control group (P <0.05). The time needed for remission was higher in the treatment group than in the control group (P <0.05). [Conclusion] Quyu Ziyin Decoction combined with prednisone has significant curative effect on systemic lupus erythematosus and is worth promoting.
