目的:探讨Cook宫颈扩张球囊应用于足月妊娠促宫颈成熟、引产的有效性及安全性。方法:选择2012年12月-2014年5月在我院住院有引产指征的且宫颈评分<6分的足月孕妇,212例为研究对象,随机分为两组,106例应用Cook宫颈扩张球囊引产作为观察组,106例孕妇应用地诺前列酮栓(欣普贝生)引产作为对照组,观察两组宫颈成熟度、分娩结局、24 h内的分娩率、从引产开始至临产时间、从引产开始至分娩时间、缩宫素的使用情况、不良反应、产后出血量、产褥感染发生情况及新生儿的情况等,并分别进行比较分析。结果:观察组采用Cook宫颈扩张球囊与对照组的欣普贝生组在促宫颈成熟、临产时间、分娩结局、产褥感染发生情况及新生儿的情况比较,差异无统计学意义(P>0.05),24 h内的分娩率分别为50.57%,70.00%、缩宫素的使用情况、产后出血量、不良反应比较,差异具有统计学意义(P<0.05)。结论:Cook宫颈扩张球囊在扩张宫颈,促进宫颈成熟和提高引产成功率的效果与欣普贝生类似,同时不增加产褥感染发生率,但可以降低子宫过度刺激、胎心异常等不良反应。
Objective: To investigate the effectiveness and safety of Cook cervical dilatation balloon for full-term pregnancy promoting cervical ripening and induction of labor. Methods: A total of 212 full-term pregnant women with indications for induction of labor in our hospital from December 2012 to May 2014 were enrolled in this study. 212 pregnant women were randomly divided into two groups. 106 cases were treated with Cook cervical dilatation Infarction of the balloon as the observation group, 106 cases of pregnant women with dirprost thiazole suppository (Yan Popei) induced labor as a control group, the two groups of cervical maturation, delivery outcome within 24 h of delivery rate, from induction of labor to the time of labor , From the beginning of induction of labor to delivery time, the use of oxytocin, adverse reactions, postpartum hemorrhage, the incidence of puerperal infection and neonatal situation, and were comparative analysis. Results: There were no significant differences between the observation group and the control group in the Cook cervical dilatation balloon and the control group in promoting cervix maturation, labor time, delivery outcome, the occurrence of puerperal infection and newborns (P> 0.05). The rates of delivery within 24 h were 50.57% and 70.00% respectively. The use of oxytocin, the amount of postpartum hemorrhage and adverse reactions were statistically significant (P <0.05). Conclusions: The effect of Cook cervical dilatation balloon in expanding cervix, promoting cervical ripening and increasing success rate of induction of labor is similar to that of Hinbutsone without increasing the incidence of puerperal infection, but can reduce the adverse reactions such as uterine over stimulation and abnormal fetal heart rate .
