目的探讨医务人员手卫生管理模式,提高医务人员手卫生的依从性,预防与控制医院感染的发生。方法运用FOCUS-PDCA程序管理方法研究我院手卫生执行过程中存在的不足,分析查找原因并制定解决方案,同时督促落实情况。分析比较干预前后手卫生依从率的变化。结果实施FOCUS-PDCA管理方法后,手卫生依从率从干预前的31.7%提升至69.9%,干预前后差异有统计学意义(P<0.05)。经干预后不同科室、不同类别专业人员及不同手卫生指征的手卫生依从率均有提高,ICU科手卫生依从率提高效果最明显,从干预前的43.3%提升至干预后的85.0%,差异有统计学意义(P<0.05);医技人员手卫生依从率提升最为明显,从干预前的7.3%提升至干预后的52.0%,差异有统计学意义(P<0.05);不同手卫生指征中直接接触患者前提升最明显,从干预前的20.75%提升至干预后的59.08%,差异有统计学意义(P<0.05)。结论 FOCUS-PDCA程序管理方法对提高手卫生依从率效果明显,可改善医务人员手卫生状况,有一定的推广应用价值。
Objective To explore the hand hygiene management mode of medical staff, improve the hand hygiene compliance of medical staff, and prevent and control the occurrence of nosocomial infection. Methods The method of FOCUS-PDCA management was used to study the shortcomings in hand hygiene implementation in our hospital. The causes of the hand-hygiene were analyzed and the solution was found out. At the same time, supervision was put on implementation. Analysis and comparison of changes in hand hygiene compliance before and after intervention. Results After implementing FOCUS-PDCA management method, the hand hygiene compliance rate increased from 31.7% before intervention to 69.9%, with significant difference before and after intervention (P <0.05). After intervention, the hand hygiene compliance rate of different departments, different categories of professionals and different hand hygiene indications increased. ICE hand hygiene compliance rate was the most obvious, from 43.3% before intervention to 85.0% after intervention, The difference was statistically significant (P <0.05); medical and technical personnel hand hygiene compliance rate increased most significantly from 7.3% before intervention increased to 52.0% after intervention, the difference was statistically significant (P <0.05); different hand hygiene The most obvious sign of direct contact with patients before the promotion, from 20.75% before intervention increased to 59.08% after intervention, the difference was statistically significant (P <0.05). Conclusion The FOCUS-PDCA program management method has obvious effect on improving hand hygiene compliance rate, which can improve the hand hygiene status of medical staff and has certain promotion and application value.
