Objective: This paper aims to exlore the treatment effect of an anterior medial neurovascular interval approach to coronal shear fractures of the distal humerus.Method: This study was a prospective study.There were 2 females,aged from 30-64 years,mean 47 year.Fractures were caused by falling off the bicycle.The time between injury and operation was 1-2 days,mean,1.5 days.Two patients with coronal shear fracture of the distal humerus were treated with open reduction and internal fixation by anterior neurovascular interval approach.In 1 patient with the ulna coronoid process fracture,reduction and fixation of coronoid fracture was given through the same approach;Another patient with fracture of lateral epicondyle of the humerus,the fixation of the fracture was given through another incision.Plast splin was used to fix in two patients,function exercise was done after removal of external fixation postoperative 2-3 weeks.Results:Without intraoperative and postoperative neurological,vascular complications,with-out infection,the fracture was united,12 months after surgery the patient returned to her original work without pain,and normal range of motion of the elbow and forearm rotation;the X-ray films showed an excellent fracture union and no signs of heterotopic ossification and no traumatic arthri-tis.The average ange of flexion and extension activities of elbow was135°,The average range of activities of forearm rotation was 155°,According to the Mayo elbow function evaluation stan-dards: 100 scores,excellent.Conclusion: Applying an anterior neurovascular interval approach of the elbow in the treatment of shear fracture of the articular surface of the distal humerus,particularly the trochlea of the humerus,it can reduce stripping of soft tissue,the exposing enough,easy to direct reduction and fixation of the fracture,reduce the incidence of complications.It can be used alone or combined with other surgical approach,obtaining to good treatment results.
