Documentation of increased renal sympathetic outflow in essential hypertension (with the isotope dilution, noradrenaline spillover technique), coupled with BP lowering from renal surgical denervation in experimental hypertension models,provides a theoretical basis for selective renal sympathetic denervation in resistant hypertension.Laparoscopic surgery is not feasible; as the nerves lie in the renal artery wall, bilateral renal artery transection and re-anastomosis would be needed.My colleagues and I, working with ARDIAN corporation USA, have developed an alternative, and successful technique, utilising intra-luminal radiofrequeney energy delivery, which has been shown to safely denervate the kidneys,and to lower blood pressure in patients with previously uncontrollable essential hypertension.
