Objective To discuss the surgical results of improved en bloc ectomy of semi-articular process and vertebral plate for the treatment of thoracic canal stenosis(TCS).Methods All the 23 cases of TCS treated with improved en bloc ectomy of semi-articular process and vertebral plate from August 2001 to August 2007 were studied retrospectively, including 15 males and 8 females, with an average of 54.8 years old(ranged25 to 71 years old).The Japanese Orthopaedics Association(JOA)standard were used to evaluate for all the 23 patients with thoracic myelopathy.Results A total of 85 vertebral plate with an average of 3.7(ranged 3 to 7)were resected in 23 cases.Intraoperative teared dura(7 patients)was repaired and postoperative cerebrospinal fluid leakage(4 patients)heal after active treatment.Superficial wound infection(1 patients)was cured.The mean follow-up duration was 21 months(10 to 74 months).The mean JOA score increased from4.027±1.838 preoperatively to 8.347±2.555 during followed-up.The difference was significant (t=-19.431, P=0.000<0.01).There were excellent 8, good 9, improved 4, and unchanged 2.The excellent and good rate was 73.9%(17/23),and the total effective rate was91.3%(21/23).Conclusion Improved en bloc ectomy of semi-articular process and vertebral plate for the treatment of TCS has the advantages of rapid manipulation, thorough decompression, certainly effects, etc.
