Various tachyarrhythmias have become everyday challenge for clinicians caring for infants, children and adults with repaired or palliated congenital heart disease. The risk of atrial or ventricular tachycardia increases with the time from surgical intervention, especially in patients with complex heart defects.Frequently, tachyarrhythmia can be successfully managed with medical antiarrhythmic therapy or surgical intervention for significant residual heart disease. In general, catheter ablation is reserved for individuals with recurrent tachyarrhythmia refractory to medical therapy.Catheter ablation may be hindered by difficult vascular access, complex intra-cardiac anatomy and multiple arrhythmia substrates. We present our experience with catheter ablation of incisional atrial tachycardia and discuss general approach strategies in these patients.
