住院患者肾功能衰竭患病情况及漏诊分析

目的了解综合性医院住院患者中肾功能衰竭的患病情况,分析肾功能衰竭的发病率、病因构成以及各科室对肾功能衰竭诊断和漏诊的分布情况,以利于提高相关学科医师对肾功能衰竭的认识水平。方法收集1年内医院所有住院患者的血液生化检查结果,筛选出血肌酐升高超出正常值范围的患者病历资料,进行回顾性分析。结果共有1402位肾功能衰竭患者,占同期住院患者的3.4%,男性占63.4%(889例),女性占36.6%(513例)。肾功能衰竭患者的住院科室分布,肾脏内科占42.2%(592例),其他内科系统占25.7%(360例),外科系统占32.1%(450例)。急性肾功能衰竭占26.8%(376例),慢性肾功能衰竭占73.2%(1026例)。肾功能衰竭漏诊率:内科系统为36.7%;外科系统为71.2%。肾脏内科会诊率:内科系统为33.4%;外科系统为20.8%。结论肾功能衰竭患者普遍分布在非肾脏内科系统,肾功能衰竭漏诊在非肾脏内科系统和外科系统发生率非常高,尤其是在外科系统。 Objective To understand the prevalence of renal failure in inpatients in general hospitals and to analyze the incidence of renal failure, the cause of the disease and the distribution of diagnosis and misdiagnosis of renal failure in all departments in order to facilitate the improvement of renal failure The level of awareness. Methods The blood biochemical test results of all hospitalized patients in the hospital for 1 year were collected, and the records of patients with elevated serum creatinine beyond the normal range were screened for retrospective analysis. Results A total of 1402 patients with renal failure accounted for 3.4% of hospitalized patients in the same period, accounting for 63.4% (889 cases) of men and 36.6% (513 cases) of women. There were 42.2% (592 cases) of renal diseases, 25.7% (360 cases) of other medical systems and 32.1% (450 cases) of surgical systems in patients with renal failure. Acute renal failure accounted for 26.8% (376 cases), chronic renal failure accounted for 73.2% (1026 cases). The rate of missed diagnosis of renal failure was 36.7% in the medical system and 71.2% in the surgical system. Nephrology consultation rate: medical system was 33.4%; surgical system was 20.8%. Conclusions Patients with renal failure are commonly found in non-renal medical systems. The incidence of missed diagnosis of renal failure in non-renal medical and surgical systems is very high, especially in the surgical system.
