目的研究血液净化治疗技术在急诊重度有机磷中毒抢救中的应用效果,探讨其可行性及安全性。方法以2012年1月—2014年1月收治的36例重度有机磷中毒患者为观察组,以2010年1月—2011年12月收治的33例重度有机磷中毒患者为对照组。所有患者采用常规治疗,观察组在此基础上接受血液净化治疗。观察两组治疗情况、并发症发生情况及抢救成功率。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果观察组阿托品用量、自主呼吸恢复时间、意识恢复时间、住院时间[(45.45±21.27)mg、(2.14±0.89)d、(8.13±2.65)h、(11.26±2.57)d]与对照组比较,差异均有统计学意义(t=2.27、6.75、12.28、8.91,均P<0.05)。观察组呼吸衰竭、肝损伤、肾损伤的发生率(2.78%、8.33%、8.33%)均低于对照组,差异均有统计学意义(χ2=4.48、5.43、10.78,均P<0.05)。观察组抢救成功率为97.22%,对照组为78.79%,比较差异有统计学意义(χ2=5.71,P<0.05)。结论血液净化治疗提高了急诊重度有机磷中毒患者的抢救成功率,且安全性好,可推广应用。
Objective To study the application of blood purification in the emergency treatment of severe organophosphate poisoning and to explore its feasibility and safety. Methods From January 2012 to January 2014, 36 patients with severe organophosphate poisoning were selected as the observation group and 33 patients with severe organophosphate poisoning admitted from January 2010 to December 2011 as the control group. All patients treated with conventional treatment, the observation group on the basis of blood purification treatment. Observation of two groups of treatment, complications and rescue success rate. Measurement data using t test, count data using χ2 test, P <0.05 for the difference was statistically significant. Results The amount of atropine, spontaneous breathing recovery time, consciousness recovery time, hospital stay [(45.45 ± 21.27) mg, (2.14 ± 0.89) d, (8.13 ± 2.65) h, (11.26 ± 2.57) d] in the observation group were significantly higher than those in the control group , The differences were statistically significant (t = 2.27,6.75,12.28,8.91, all P <0.05). The incidence of respiratory failure, liver injury and renal injury in the observation group (2.78%, 8.33%, 8.33%) were lower than those in the control group (χ2 = 4.48,5.43,10.78, all P <0.05). The successful rate of rescue in the observation group was 97.22% and that in the control group was 78.79%, with significant difference (χ2 = 5.71, P <0.05). Conclusion Blood purification treatment can improve the success rate of emergency treatment of patients with severe organophosphorus poisoning, and has good safety and can be popularized and applied.
