目的探讨短期应用人类重组生长激素(rh-GH)对法洛四联症(TOF)病儿根治术后免疫功能的影响。方法30例TOF病儿分为常规组(20例)和rh-GH组(10例,rh-GH0·2U/kg,每周3次,皮下注射,连续应用4周);动态观察免疫球蛋白、补体、淋巴细胞表型和细胞因子的变化。结果应用rh-GH后,异常升高的IgG、IgM、C3、C4、CD8+和CD19+于治疗后第1~3周开始下降,异常下降的IgA、CD3+、CD4+、CD4+/CD8+、CD3+/HLA-DR+和CD3+/CD(16+56)于治疗后1~2周开始逐渐升高,且上述指标均于治疗后4周恢复到对照组水平。血浆和外周血单个核细胞体外产生的白细胞介素-6和肿瘤坏死因子-α于治疗后1~2周开始逐渐下降,于治疗后4周恢复到对照组水平。结论rh-GH可明显改善TOF病儿术后的免疫功能状态。
Objective To investigate the effect of short-term human recombinant growth hormone (rh-GH) on immune function in children with tetralogy of Fallot (TOF) after radical operation. Methods Thirty children with TOF were divided into two groups: conventional group (20 cases) and rh-GH group (10 cases, rh-GH0.2 U / kg, three times a week for subcutaneous injection for 4 weeks); dynamic observation of immunoglobulin , Complement, lymphocyte phenotype and cytokine changes. Results The levels of IgA, CD3 +, CD4 +, CD4 + / CD8 +, CD3 + / HLA- 1 in the group of IgG, IgM, C3, C4, CD8 + and CD19 + DR + and CD3 + / CD (16 + 56) gradually increased from 1 week to 2 weeks after treatment, and the above indexes returned to the level of control group 4 weeks after treatment. Interleukin-6 and tumor necrosis factor-α produced by plasma and peripheral blood mononuclear cells in vitro gradually decreased from 1 to 2 weeks after treatment, and returned to control level at 4 weeks after treatment. Conclusion rh-GH can significantly improve the immune status of children with TOF disease.
