目的通过脉冲组织多普勒(PW-TDI)技术测量Tei指数评价糖尿病患者的右心室功能。方法糖尿病患者33例,健康对照组34例,常规超声心动图检查,同时应用PW-TDI技术于心尖四腔观获取三尖瓣环的PW-TDI频谱图,并结合标Ⅱ导联分别测量右心室等容收缩时间(ICT)、射血时间(ET)、等容舒张时间(IRT),计算出右心室Tei指数,所有数据采用SAS9.0进行分析加以比较。结果 (1)糖尿病组的Tei指数明显增大,与对照组比较差异有统计学意义(P<0.01);(2)单元线性分析表明:正常对照组中,Tei指数与年龄心率均无相关性(P>0.05),糖尿病患者中,Tei指数与年龄呈正相关(P<0.05),而与心率无相关性(P>0.05)。结论采用PW-TDI技术所测的Tei指数简便、敏感、准确性高,能较全面准确地评价糖尿病患者的右心室功能情况。
Objective To evaluate the right ventricular function in diabetic patients by measuring the Tei index by pulsed tissue Doppler (PW-TDI) technique. Methods Thirty-three patients with diabetes mellitus and 34 healthy controls were enrolled in this study. Conventional echocardiography and PW-TDI were used to acquire the PW-TDI spectra of the tricuspid annulus in apical four-chamber view. Ventricular isovolumic contraction time (ICT), ejection time (ET) and isovolumic relaxation time (IRT) were calculated to calculate the right ventricular Tei index. All data were analyzed by SAS9.0 for comparison. Results (1) The Tei index of diabetic group increased significantly compared with that of control group (P <0.01). (2) Unit linear analysis showed that there was no correlation between Tei index and age and heart rate in normal control group (P> 0.05). There was a positive correlation between Tei index and age in diabetic patients (P <0.05), but no correlation with heart rate (P> 0.05). Conclusion The Tei index measured by PW-TDI technique is simple, sensitive and accurate. It can evaluate the right ventricular function in diabetic patients comprehensively and accurately.
