目的探讨亚低温对幼猪外伤性脑损伤(traum atic brain in jury,TB I)的保护作用。方法将幼猪TB I模型(液压法)随机分为常温对照组(颅温37.5℃~38.5℃;n=10)和亚低温组(34℃±0.2℃;n=10)。检测脑脊液中特异性烯醇化酶(NSE)、S-100蛋白(S-100)、磷酸肌酸激酶脑型同工酶(CK-BB)水平变化,监测颅内压的变化以及病理检查脑梗塞体积及脑损伤积分。结果常温组TB I后48 h颅内压达高峰,峰值(38.7±6.7)mmHg,亚低温组颅内压均维持在较低水平,各时点指标均比常温对照组明显降低(P<0.01);TB I后24 h、48 h、72 h亚低温组脑脊液NSE水平较常温对照组明显降低(P<0.01);亚低温组脑脊液S-100、CK-BB测定值亦明显低于常温对照组各个时点的水平(P<0.05、P<0.001);脑损伤积分及脑梗塞体积亚低温组分别为(10.2±3.0)mm3和(38.4±23.5)mm3,显著少于常温对照组(16.4±4.2)mm3、(85.6±18.2)mm3;P<0.05。结论颅内压、脑脊液生化指标物NSE、S-100、CK-BB水平,病理分析证实亚低温对幼猪TB I具有较好的脑保护作用。
Objective To investigate the protective effect of mild hypothermia on traumatic brain injury (TBI) in young pigs. Methods TB I model (hydraulic method) of young pigs were randomly divided into normal temperature control group (cranial temperature 37.5 ℃ ~ 38.5 ℃; n = 10) and mild hypothermia group (34 ℃ ± 0.2 ℃; n = 10). The levels of specific enolase (NSE), S-100 (S-100) and creatine phosphokinase (CK-BB) in cerebrospinal fluid were measured and the changes of intracranial pressure and the changes of intracranial pressure Volume and brain injury scores. Results The intracranial pressure peaked at 48 h after TBI in normal temperature group (38.7 ± 6.7 mmHg), while the intracranial pressure in mild hypothermia group remained at a low level at all time points (P <0.01) ). The levels of NSE in CSF in 24 h, 48 h, 72 h TBI group were significantly lower than those in normal control group (P <0.01). The values of CSF S-100 and CK-BB in mild hypothermia group were also significantly lower than those in normal temperature control group (10.2 ± 3.0) mm3 and (38.4 ± 23.5) mm3, respectively, which were significantly lower than those in the normal temperature control group (16.4, P <0.05, P < ± 4.2) mm3, (85.6 ± 18.2) mm3; P <0.05. Conclusions Intracranial pressure and cerebrospinal fluid biochemical markers NSE, S-100, CK-BB levels, pathological analysis confirmed that mild hypothermia in young pigs TB I have a good brain protective effect.
