目的应用细胞图像分析术(ICM-DNA)对大宗病例非霍奇金淋巴瘤(NHL)进行DNA参数的检测,寻找对NHL的诊断、鉴别诊断和预后有指导意义的参考指标。方法对210例NHL(WHO2001新分类)病例及30例良性增生淋巴组织(RH)行改良的Feulgen染色并作ICM-DNA检测。结果NHL平均DNA含量为7.01~10.77 pg;SPF为10.97%~30.87%,异倍体率7.76%~17.76%,均高于对照组(p≤0.01);NHL二倍体百分含量为16.03%~70.90%,低于对照组(p≤0.01);NHL侵袭组、高侵袭组的平均DNA含量、SPF、异倍体率均高于惰性组(p≤0.01);FL-II、III级的平均DNA含量、SPF、异倍体率均高于FL-I级(p≤0.01)。结论ICM-DNA检测中,平均DNA含量、SPF、异倍体率、二倍体百分含量对良恶淋巴组织增生的鉴别诊断、NHL惰性或侵袭性预后判断均为有意义的参考指标;但对侵袭性高低无提示,FL是相对异质化群体,不能简单归为惰性。
Objective To detect the DNA parameters of non-Hodgkin’s lymphoma (NHL) in large cases by using cell image analysis (ICM-DNA), and to find the reference index for the diagnosis, differential diagnosis and prognosis of NHL. Methods 210 cases of NHL (WHO2001 new classification) cases and 30 cases of benign hyperplasia of lymphoid tissue (RH) modified Feulgen staining and ICM-DNA detection. Results The average DNA content of NHL was 7.01-10.77 pg, the SPF was 10.97% -30.87%, the aneuploidy rate was 7.76% -17.76%, higher than that of the control group (p≤0.01). The NHL diploid percentage was 16.03% ~ 70.90%, lower than the control group (p≤0.01); the average DNA content, SPF and aneuploidy rate of NHL invasion group and high invasion group were higher than those of the inactive group (p≤0.01); FL-II, The average DNA content, SPF, aneuploidy rate were higher than FL-I level (p≤0.01). Conclusion ICM-DNA detection, the average DNA content, SPF, aneuploidy rate, diploid percentage of differential diagnosis of benign and malignant lymph node hyperplasia, NHL inert or invasive prognostic judgment are significant reference indicators; No indication of the level of invasion, FL is relatively heterogeneous groups, can not be simply classified as inert.
