目的 探讨先天性单纯阴茎下曲的治疗方法和疗效。 方法 采用阴茎皮肤脱套、阴茎背侧白膜折迭、尿道游离以及尿道成形等方法治疗先天性单纯阴茎下曲 2 3例。年龄 1～ 2 1岁 ,平均 7岁。其中Ⅰ型 2例 ,Ⅱ型 11例 ,Ⅲ型 6例 ,Ⅳ型 4例。 结果 Ⅱ型和Ⅲ型 17例经阴茎皮肤脱套 ,Ⅳ型 1例经阴茎皮肤脱套和白膜折迭 ,Ⅰ型 1例和Ⅳ型 2例经阴茎皮肤脱套、尿道游离和白膜折迭 ,Ⅰ型和Ⅳ型各 1例经尿道成形 ,下曲均完全矫正。术中尿道损伤 2例 ,1例术后并发感染、尿瘘 ,8个月后修补治愈。 1例术后阴茎头感刺痛 ,3周后消失。 16例获得随访 7～ 5 7个月 ,平均 2 4个月 ,除1例阴茎头略向腹侧弯曲 (<15°)外 ,15例阴茎完全伸直 ,未见下曲复发。 结论 先天性单纯阴茎下曲需根据不同病因采用不同治疗方法。阴茎皮肤脱套、背侧白膜折迭术并发症少 ,疗效满意 ,是先天性单纯阴茎下曲的基本治疗方法。
Objective To investigate the treatment and efficacy of congenital simple penile song. Methods Twenty - three congenital simple penile subchromosia were treated with penile skin debridement, penile dural fold, urethral detachment and urethroplasty. Aged 1-2 years old, an average of 7 years old. There were 2 cases of type Ⅰ, 11 cases of type Ⅱ, 6 cases of type Ⅲ and 4 cases of type Ⅳ. Results There were 17 cases of type Ⅱ and Ⅲ in the penis, 1 case of type Ⅳ in 1 case and type Ⅳ in 2 cases of penis skin debridement, urethral dissociation and albuginea Diego, type Ⅰ and type Ⅳ in 1 case of urethral formation, the next song are completely corrected. Intraoperative urethral injury in 2 cases, 1 case of postoperative infection, urinary fistula, 8 months after the cure. Postoperative penis headache sensation in 1 case, disappeared after 3 weeks. Sixteen patients were followed up for 7-57 months (average 24 months). Except 1 case with glans penis slightly ventral bending (<15 °), 15 cases of penis were completely straightened and no recurrence was found. Conclusion Congenital simple penile curvature according to different causes of different treatment methods. Penis skin debridement, dorsal albuginea fold less complications, the effect is satisfactory, is the basic treatment of congenital simple penile song.
