目的分析某基层医院2015年1—9月处方点评结果及不合理处方原因。方法选取某基层医院2015年1—9月各1d的门诊急诊西药房处方(麻精药品处方除外)5312张,并对处方合格率及不合理处方原因进行分析。结果 2015年1—9月处方合格率均>95%,总体合格率为95.8%;5312张处方中不合理处方223张(4.20%),其中以适应证不适宜处方居多,占47.09%,其次为药品遴选不适宜及无特殊情况给药时间>7d处方。结论该基层医院2015年1—9月处方合格率基本达标,但医师的药品掌握水平及正确书写处方的意识亟待加强。
Objective To analyze the results of prescription reviews and the causes of unreasonable prescriptions in a primary hospital from January to September, 2015. Methods 5312 outpatient emergency western pharmacy prescriptions (except for prescriptions of cannabinoids) in a grass-roots hospital from January to September in 2015 were selected, and the analysis of the pass rate of prescriptions and the reasons of unreasonable prescriptions. Results From January to September of 2015, the passing rates of prescriptions were all higher than 95% and the overall pass rate was 95.8%; 223 (4.20%) of 5312 prescriptions were irrational, of which 47.09% were mostly inappropriate indications, accounting for 47.09% Not suitable for drug selection and no special circumstances for the time> 7d prescription. Conclusion The primary hospital’s compliance rate of prescription from January to September in 2015 basically reached the standard, but the physician’s mastery of medicines and the awareness of correctly writing prescriptions need to be strengthened.
