目的对过氧化物酶体增殖物激活受体γ(PPARγ)基因外显子6与多囊卵巢综合征(PCOS)的关系进行系统评价。方法检索中英文数据库2016年6月30日之前发表的关于PPARγ基因多态性和PCOS关系的文献资料,应用RevMan5.3软件进行系统评价。采用随机效应模型或固定效应模型计算效应指标:OR值和95%CI进行亚组分析和敏感性分析,用Egger’s法通过stata12.0软件定量评估发表偏倚。结果 PPARγ基因外显子6 His447His多态性与PCOS无明显关系。按种族进行亚组分析,在黄种人中共显性模型CC vs CT:Z=2.03(P=0.04);共显性模型CC vs TT:Z=2.88(P=0.004);显性模型:Z=2.54(P=0.01);超显性模型:Z=2.50(P=0.01);隐性模型:Z=2.58(P=0.010);等位基因模型:Z=3.00(P=0.003)。在白种人中共显性模型CC vs CT:Z=2.03(P=0.04)。结论黄种人PPARγ基因His447His的多态性与PCOS易感性有关;且等位基因C和T分别为PCOS的保护和易感因素。白种人中PPARγ基因His447His的多态性可能与PCOS有关,且等位基因C可能是PCOS的易感因素。
Objective To systematically evaluate the relationship between exon 6 of peroxisome proliferator - activated receptor γ (PPARγ) gene and polycystic ovary syndrome (PCOS). Methods The Chinese and English databases were searched before and after June 30, 2016, and the literature about the relationship between PPARγ gene polymorphism and PCOS was analyzed by using RevMan5.3 software. Random effect model or fixed effect model was used to calculate the effect index: OR value and 95% CI for subgroup analysis and sensitivity analysis, and Egger’s method was used to quantitatively evaluate publication bias with stata12.0 software. Results The polymorphism of His447His in exon 6 of PPARγ gene had no significant relationship with PCOS. CC vs TT: Z = 2.88 (P = 0.004); dominant model: Z = 2.54 (P = 0.01); Overtype model: Z = 2.50 (P = 0.01); Recessive model: Z = 2.58 (P = 0.010); Allele model: Z = 3.00 (P = 0.003). In the Caucasian co-dominant model CC vs CT: Z = 2.03 (P = 0.04). Conclusion The genetic polymorphism of His447His in PPARγ gene is related to the susceptibility to PCOS. The alleles C and T are the protective and susceptible factors of PCOS, respectively. Polymorphism of His447His in PPARγ gene may be related to PCOS in Caucasian, and allele C may be a predisposing factor in PCOS.
