目的探讨高危型人乳头瘤病毒(HPV)检测在液基薄层细胞学(TCT)报告为ASC-US患者中的作用。方法选取2012年5月31日-2012年11月30日在本院妇科门诊行宫颈筛查妇女2932人,共检出ASC-US患者235人,ASC-US患者行高危型HPV检测,对ASC-US、HPV(-)根据妇科检查、临床表现选择性行阴道镜加宫颈活组织检查,对ASC-US、HPV(+)全部行阴道镜加宫颈活组织检查。结果 ASC-US患者行高危型HPV检测,HPV阳性67人,占28.5%(67/235),HPV阴性168人,占71.5%(168/235)。当ASC-US、HPV(+)时80.6%(54/67)为良性病变,4.5%(3/67)为低度病变,11.9%(8/67)为高度病变,3%(2/67)为宫颈癌;当ASC-US、HPV(-)时根据妇科检查、病史选出44.6%(75/168)人行阴道镜检查加宫颈活检。97.3%(73/75)为良性病变,只有3.7%(2/75)为低度病变。结论对ASC-US患者采用HPV检测进行分流管理是一种快速有效方法,应逐步推广和应用。
Objective To investigate the role of high-risk human papillomavirus (HPV) in the detection of ASC-US by liquid-based thin-layer cytology (TCT). Methods A total of 2,932 women were screened by cervix in our gynecology clinic from May 31, 2012 to November 30, 2012. A total of 235 ASC-US patients were detected, ASC-US patients were tested for high-risk HPV, -US, HPV (-) According to gynecological examination, clinical manifestations of selective colposcopy plus cervical biopsy, ASC-US, HPV (+) all colposcopy plus cervical biopsy. Results ASC-US patients underwent high-risk HPV testing. There were 67 positive HPVs (28.5%, 67/235) and HPV negative 168 (71.5%, 168/235). When ASC-US and HPV (+) were used, 80.6% (54/67) were benign lesions, 4.5% (3/67) were low-grade lesions, 11.9% (8/67) ) For cervical cancer; when ASC-US, HPV (-) based on gynecological examination, medical history selected 44.6% (75/168) colposcopy plus cervical biopsy. 97.3% (73/75) were benign and only 3.7% (2/75) were low-grade. Conclusion It is a quick and effective method to use the HPV test to divert management in ASC-US patients, which should be gradually popularized and applied.
