Objective: To analyze the correlation between white matter chronic ischemia and blood lipid levels. Methods: The chronic ischemic rats were divided into groups according to the magnetic resonance (MR) findings. The levels of total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL) and their isoenzyme 2 HDL2, HDL3, LDL, apoA1, apoB10 0 and their ratio (A / B), as well as blood pressure , The relevance of smoking. Results: The mean levels of TC, TG and apoB10 0 in white matter with chronic cerebral ischemia group were significantly higher than those in normal group (P 0 05), HDL, HDL 2 and HDL 3 were lower than those in normal group (P 0 05) The normal group (P <0 05), apoA1 and A / B lower than the normal group (P <0 05). HDL2 in subcortical arteriosclerotic encephalopathy group was lower than that in normal group (P <0.05), but TC, TG, HDL, HDL3, LDL, apoA1, apoB10 0 and A / B were similar to chronic cerebral ischemia group. Hypertension frequency was significantly higher in chronic cerebral ischemia group than in normal group (P <0.05), while frequency of smoking was similar in both groups (P> 0.05). Logistic regression analysis suggested that LDL and hypertension were the independent risk factors of chronic white matter ischemia. CONCLUSIONS: Any increase of LDL in plasma and decrease of HDL2, apoA1, A / B in patients over 50 years old may be considered as potential risk factors of cerebrovascular disease, especially when accompanied by hypertension
