目的检测系统性红斑狼疮(SLE)患者外周血滤泡辅助性T细胞(Tfh)的水平,探讨Tfh与SLE的发生及病情活动性的关系。方法 45例SLE患者和30例健康对照者,采用流式细胞术检测PBMC中Tfh水平,ELISA法检测血清中IL-21,IL-17水平,实时荧光定量RT-PCR检测PBMC中CXCR5 mRNA的表达。结果活动组SLE患者外周血Tfh,CXCR5 mRNA的表达和血清中IL-21,IL-17水平均高于非活动组和健康对照组;SLE患者外周血Tfh水平与CXCR5 mRNA的表达呈正相关,与SLEDAI亦呈正相关,与抗核抗体滴度及补体C4水平无明显相关性,与补体C3水平呈负相关。其中CXCR5 mRNA的表达水平与SLEDAI呈高度的正相关(r=0.92,P<0.01)。结论 SLE患者外周血中Tfh细胞水平明显升高,可能在其发病过程中起了一定的作用。
Objective To detect the levels of peripheral blood follicular-supporting T cells (Tfh) in patients with systemic lupus erythematosus (SLE) and to explore the relationship between Tfh and the occurrence of SLE and the activity of the disease. Methods 45 SLE patients and 30 healthy controls were enrolled in this study. Flow cytometry was used to detect Tfh levels in PBMCs. Serum levels of IL-21 and IL-17 were measured by ELISA. CXCR5 mRNA expression was detected by real-time fluorescent quantitative RT-PCR . Results The levels of Tfh and CXCR5 mRNA in peripheral blood and serum levels of IL-21 and IL-17 in patients with active SLE were higher than those in inactive and healthy controls. There was a positive correlation between Tfh and the expression of CXCR5 mRNA in SLE patients SLEDAI also showed a positive correlation with antinuclear antibody titer and complement C4 levels were not significantly correlated with the level of complement C3 was negatively correlated. The expression of CXCR5 mRNA was positively correlated with SLEDAI (r = 0.92, P <0.01). Conclusion The level of Tfh in peripheral blood of patients with SLE is significantly increased, which may play a role in the pathogenesis of SLE.
