术前明确复杂性局部发作(CPS)病因及准确定位致癫痫灶对手术实施非常重要。近代四大无创性影像技术对CPS显示出了各自的优势:XCT及MRI可良好地显示病灶形态结构改变,但对微小的结构或功能异常可被疏漏,其探测灵敏度分别为<30%及40~50%;PET及SPECT可良好显示病灶的生理、生化、代谢异常,CPS的发作间期癫痫灶血流减少,代谢降低,其探测灵敏度为60%~70%,而发作期癫痫区血流增加,代谢增加,其探测灵敏度可达92%~100%。
