目的评价亲水涂层导尿管预防脊髓损伤患者尿路感染和镜下血尿的效果。方法本研究系统地检索Cochrane Library、Joanna Briggs Institute Database、Pub Med、Embase、中国生物医学文献数据库(CMB)、万方数据库、维普数据库(VIP)和中国期刊全文数据库(CNKI),查找关于亲水涂层导尿管对脊髓损伤患者尿路感染和镜下血尿影响的随机对照研究,检索时限均从建库至2015年9月。2名研究人员独立对纳入的文献进行质量评价并提取数据。采用Rev Man 5.3软件进行数据分析。对无法进行Meta分析的结局指标,进行描述性分析。结果共纳入9篇文献,735例患者。Meta分析结果显示,亲水涂层导尿管对减少脊髓损伤患者尿路感染发生的有效率[RR=0.77,95%CI(0.61,0.97),P<0.05]和降低镜下血尿发生的有效率[RR=0.75,95%CI(0.62,0.91),P<0.05]优于对照组,差异有统计学意义(P<0.05)。结论现有证据表明,亲水涂层导尿管能够降低脊髓损伤患者尿路感染的发生率和镜下血尿的发生率。鉴于上述研究数量和质量有限,需开展更多高质量的随机对照试验予以验证。
Objective To evaluate the effect of hydrophilic coated catheters in preventing urinary tract infection and microscopic hematuria in patients with spinal cord injury. Methods This study systematically searched Cochrane Library, Joanna Briggs Institute Database, Pub Med, Embase, China Biomedical Literature Database (CMB), Wanfang Database, VIP Database and CNKI, Coated catheter on patients with spinal cord injury urinary tract infection and microscopic hematuria randomized controlled study, search time from the construction to September 2015. Two researchers independently assessed the quality of the included literature and extracted the data. Rev Man 5.3 software for data analysis. Descriptive analysis of outcome measures that failed Meta-analysis. Results A total of 9 articles were included in 735 patients. Meta-analysis showed that hydrophilic coated catheters were effective in reducing urinary tract infection in patients with spinal cord injury [RR = 0.77, 95% CI (0.61, 0.97), P <0.05] and decreased microscopic hematuria Efficacy [RR = 0.75, 95% CI (0.62, 0.91), P <0.05] was better than the control group, the difference was statistically significant (P <0.05). Conclusion The available evidence shows that hydrophilic coated catheters can reduce the incidence of urinary tract infection and microscopic hematuria in patients with spinal cord injury. In view of the limited number and quality of the above studies, more high-quality randomized controlled trials are required.
