目的探讨趋化因子受体4(CXCR4)及Ki-67在甲状腺癌中的表达及临床意义,为判断甲状腺癌预后寻找新的指标。方法将112例甲状腺癌患者按病理类型分为未分化癌与分化型癌(乳头状腺癌、滤泡状腺癌、髓样癌),用免疫组化SP染色法检测CXCR4及Ki-67在各型甲状腺癌组织中的表达,应用统计学方法分析二者的表达与肿瘤病理类型、淋巴结转移、患者年龄及性别的关系。结果 CXCR4在各型甲状腺癌组织中均为阳性或强阳性表达,分化型癌组织中CXCR4强阳性表达率与未分化癌比较,差异有统计学意义(P<0.01);有淋巴结转移和无淋巴结转移的甲状腺癌组织中CXCR4强阳性表达率比较,差异有统计学意义(P<0.01)。不同病理类型甲状腺癌组织中Ki-67增殖指数比较,差异有统计学意义(P<0.05);有淋巴结转移和无淋巴结转移的甲状腺癌组织中,Ki-67增殖指数比较差异亦有统计学意义(P<0.01)。CXCR4及Ki-67在不同性别及不同年龄的甲状腺癌组织中的表达比较,差异均无统计学意义(P>0.05)。线性相关分析显示,甲状腺癌组织中CXCR4和Ki-67的表达呈正相关(P<0.01)。结论 CXCR4及Ki-67在甲状腺癌组织中的表达与病理类型、淋巴结转移密切相关,可作为甲状腺癌患者判断预后的指标,与患者的年龄、性别无关。
Objective To investigate the expression and clinical significance of chemokine receptor 4 (CXCR4) and Ki-67 in thyroid carcinomas and to find new indexes for judging the prognosis of thyroid carcinoma. Methods 112 patients with thyroid cancer were divided into undifferentiated carcinoma and differentiated carcinoma (papillary adenocarcinoma, follicular adenocarcinoma and medullary carcinoma) according to the pathological type. The expressions of CXCR4 and Ki-67 were detected by immunohistochemical SP staining Various types of thyroid cancer, the expression of statistical methods used to analyze the expression of the two tumor pathological types, lymph node metastasis, patient age and gender. Results CXCR4 was positive or strongly positive in all types of thyroid cancer. The positive rate of CXCR4 in differentiated carcinoma was significantly higher than that in undifferentiated carcinoma (P <0.01). There was lymph node metastasis and no lymph node metastasis The positive rates of CXCR4 in metastatic thyroid cancer tissues were significantly different (P <0.01). Ki-67 proliferation index in different pathological types of thyroid cancer was significantly different (P <0.05). There was also a significant difference in Ki-67 proliferation index between thyroid carcinoma with lymph node metastasis and without lymph node metastasis (P <0.01). The expression of CXCR4 and Ki-67 in thyroid cancer tissues of different genders and different ages had no statistical significance (P> 0.05). Linear correlation analysis showed that the expression of CXCR4 and Ki-67 in thyroid cancer was positively correlated (P <0.01). Conclusion The expression of CXCR4 and Ki-67 in thyroid cancer is closely related to the pathological type and lymph node metastasis. It can be used as an index to judge the prognosis of patients with thyroid cancer, which has nothing to do with the patient’s age and sex.
