目的研究侵袭性牙周炎(A_gP)患者龈沟液中牙龈卟啉单胞菌(Porphyromonas gingivalis,P_g)与齿垢密螺旋体(Treponema denticola,Td)对有机酸的影响。方法对2012年3月至2015年2月河南大学第一附属医院诊治的40例侵袭性牙周炎患者(A_gP组)和体检结果为牙周健康的40名志愿者(健康对照组)进行研究。选择受检者口腔同一象限中一颗牙齿的颊侧中心点作为取样点,收集龈沟液,记录菌斑指数(PLI)、探诊深度(PD)、龈沟出血指数(SBI)和附着丧失(AL),采用毛细管电泳仪(CE)检测样本有机酸浓度。P_g和Td检测方法为聚合酶链式反应(PCR)。对检测出阳性的扩增目的条带灰度值进行定量分析,采用扩增条目的条带和DNA分子量标准中最接近条带的单位面积内的灰度比值表示目的基因的相对量,将其定为PCR产物量。结果 A_gP组PLI、PD、AL、有机酸浓度、P_g/Td检出率、P_g/Td产物量等均高于健康对照组(P<0.05);A_gP组P_g阳性检出点丁酸浓度明显高于阴性检出点(P<0.05),Td阳性检出点有机酸浓度均高于阴性检出点(P<0.05)。结论 A_gP患者龈沟液内有机酸浓度在一定水平上体现了P_g和Td的产物量,在A_gP的生成和发展中具有一定作用。
Objective To study the effects of Porphyromonas gingivalis (P_g) and Treponema denticola (Td) on the organic acids in gingival crevicular fluid of patients with aggressive periodontitis (A_gP). Methods Forty patients with aggressive periodontitis (A_gP group) and 40 periodontal healthy volunteers (healthy control group) who were diagnosed and treated at the First Affiliated Hospital of Henan University from March 2012 to February 2015 were studied. . The buccal center of one tooth in the same quadrant of oral cavity was selected as the sampling point. Gingival crevicular fluid was collected and the plaque index, probing depth (PD), sulcus bleeding index (SBI) and attachment loss (AL), the concentration of organic acids in the sample was determined by capillary electrophoresis (CE). P_g and Td detection method for the polymerase chain reaction (PCR). The positive value of amplification bands was quantitatively analyzed, and the relative amount of the target gene was expressed as the ratio of the gray level in the unit area of the band closest to the band of the amplified product and the DNA molecular weight, Set as the amount of PCR product. Results The concentrations of PLI, PD, AL, organic acids, P_g / Td and P_g / Td in A_gP group were significantly higher than those in healthy control group (P <0.05) At the negative detection point (P <0.05), the concentrations of organic acids at the positive Td detection point were higher than those at the negative detection point (P <0.05). Conclusions The concentrations of organic acids in gingival crevicular fluid of patients with A_gP show the amount of P_g and Td at a certain level, which play a role in the generation and development of A_gP.
