目的:探讨鼻咽血管纤维瘤(JNA)中乙酰肝素酶(HPA)、碱性成纤维细胞生长因子(bFGF)、血管内皮细胞生长因子(VEGF)的表达及其诱导血管生成在肿瘤扩展性生长中的作用。方法:应用免疫组织化学法检测30例JNA组织和20例下鼻甲组织中HPA、bFGF、VEGF和CD105的表达,以CD105的染色计算新生血管的微血管密度(MVD),结合临床分期进行分析。结果:JNA组织中HPA、bFGF及VEGF的阳性表达率高于下鼻甲组织(P<0.05);Ⅲ~Ⅳ期JNA组织HPA、bFGF及VEGF的阳性表达率明显高于Ⅰ~Ⅱ期(P<0.05);bFGF和VEGF的表达均与HPA呈正相关(r=0.499、0.582,均P<0.05);JNA组织中HPA、bFGF共阳性表达组的MVD值显著高于单一阳性及共阴性表达组(P<0.05);HPA、VEGF共阳性表达组的MVD值亦高于单一阳性及共阴性表达组(P<0.05)。结论:HPA可通过释放bFGF及VEGF来诱导血管生成以促进肿瘤扩展性生长,针对其进行靶向治疗,抑制肿瘤血管生成,将是JNA辅助治疗的一个新方向。
Objective: To investigate the expression of heparanase (HPA), basic fibroblast growth factor (bFGF) and vascular endothelial growth factor (VEGF) in nasopharyngeal angiofibroma (JNA) and the relationship between angiogenesis and tumor expansion The role of growth. Methods: The expressions of HPA, bFGF, VEGF and CD105 in 30 cases of JNA tissue and 20 cases of inferior turbinate tissue were detected by immunohistochemistry. The microvessel density (MVD) of neovascularization was calculated by CD105 staining and analyzed with clinical staging. Results: The positive expression rates of HPA, bFGF and VEGF in JNA tissues were higher than that in inferior turbinate tissues (P <0.05). The positive expression rates of HPA, bFGF and VEGF in JNA tissues in stage Ⅲ ~ Ⅳ were significantly higher than those in stage Ⅰ ~ Ⅱ (P < 0.05). The expression of bFGF and VEGF were positively correlated with HPA (r = 0.499,0.582, all P <0.05). The MVD of HPA and bFGF positive group in JNA group were significantly higher than that in single positive and negative group P <0.05). The MVD of HPA and VEGF co-positive expression group was also higher than that of single positive and negative co-expression group (P <0.05). CONCLUSION: HPA can induce angiogenesis by releasing bFGF and VEGF to promote tumor growth. Targeted therapy and inhibition of tumor angiogenesis will be a new direction of adjuvant therapy with JNA.
