目的　评价含纤维铂金弹簧圈合用聚乙烯醇颗粒 (PVA )行选择性支气管动脉栓塞(SBAE)治疗大咯血的疗效。方法　 2 3例患者 ,经微导管用注射器推入含纤维铂金弹簧圈及PVA行SBAE ,术前均行纤维支气管镜明确出血部位 ,术后随诊 2～ 18个月以评价疗效。结果　即时止血成功率为 95 .65 % ,近期复发率为 9.5 2 % ,中远期复发率为 0 ,并发症少而轻 ,对症治疗后缓解。结论　应用含纤维铂金弹簧圈及PVA行SBAE治疗大咯血 ,成功率高 ,适应症广 ,可避免开胸手术 ,是一种治疗大咯血的理想方法。 Objective To evaluate the curative effect of fibrin coil with polyvinyl alcohol particles (PVA) on selective hemoptysis with selective bronchial artery embolization (SBAE). Methods Twenty-three patients underwent microsurgery with a fiberoptic coil and a SBAE PVA catheter. All patients underwent preoperative bronchoscopy to confirm the bleeding site. The patients were followed up for 2 to 18 months to evaluate the curative effect. Results immediate hemostasis success rate was 95.65%, the recent recurrence rate was 9.5 2%, the long-term recurrence rate was 0, less and less complications, symptomatic relief after treatment. Conclusion The application of fibrous platinum coil and PVA SBAE treatment of hemoptysis, high success rate, wide indications, to avoid thoracotomy, is an ideal method of treatment of massive hemoptysis.
