目的研究妇科盆腔根治术后应用低分子右旋糖酐氨基酸预防静脉血栓病的可行性。方法随机对照研究276例妇科盆腔根治术后有静脉血栓病高危因素的患者,观察预防性应用低分子右旋糖酐氨基酸组及生理盐水对照组有无静脉血栓形成,并分析血常规、凝血酶原时间(PT)、凝血酶时间(TT)、活化的部分凝血酶时间(APTT)、D-二聚体、凝血因子Ⅱ促凝活性、全血黏度等指标。结果手术后,与对照组相比,预防性应用低分子右旋糖酐氨基酸组静脉血栓发生率及D-二聚体、凝血因子Ⅱ促凝活性、全血黏度明显下降(P<0.05),而凝血功能PT、TT、APTT水平无明显延长。结论预防性使用低分子右旋糖酐氨基酸可改善妇科盆腔根治术患者的凝血功能,减少血栓性疾病的发生率,提示应用低分子右旋糖酐氨基酸预防妇科盆腔根治术后静脉血栓病安全有效。
Objective To study the feasibility of using low molecular weight dextran amino acids to prevent venous thromboembolism after pelvic radical mastectomy. Methods A randomized controlled study was conducted in 276 patients with risk factors for venous thromboembolism after pelvic radical mastectomy. The preventive and therapeutic effects of low molecular weight dextran amino acids and saline control group on venous thrombosis were observed. Blood routine, prothrombin time PT), thrombin time (TT), activated partial thromboplastin time (APTT), D-dimer, coagulation factor procoagulant activity, whole blood viscosity and other indicators. Results Compared with the control group, the incidence of venous thromboembolism and D-dimer and coagulation factor Ⅱ procoagulant activity in prophylactic low molecular weight dextran amino acid group were significantly decreased (P <0.05), while the coagulation function PT, TT, APTT levels no significant prolongation. Conclusion Preventive use of low molecular weight dextran amino acids can improve coagulation function and reduce the incidence of thrombotic diseases in patients with pelvic radical mastectomy. It suggests that the application of low molecular weight dextran amino acids in the prevention of venous thromboembolism after pelvic radical operation is safe and effective.
