目的研究妊娠期高血压疾病合并心力衰竭的临床表现。方法将120例患者根据其病情分为观察组与对照组,观察组所有的患者均为妊娠期高血压合并心力衰竭症状,对照组均为单一妊娠期高血压症状,对两组患者的平均动脉压、白蛋白、总蛋白、24 h尿蛋白定量、HCT、CVT指标进行观察与对比。结果观察组的白蛋白显著低于对照组(P<0.05),而24 h尿蛋白定量又显著高于对照组(P<0.05);两组患者的总蛋白差异较小,且无统计学意义(P>0.05)。观察组的平均动脉压平均值与HCT的平均值均显著高于对照组(P<0.05),而CVT的平均值低于对照组(P<0.05)。对照组患者中无孕妇死亡病例,孕妇病死率为0.00%(0/56);对照组中新生儿死亡4例,病死率为7.14%(4/56)。观察组患者中无孕妇死亡病例,孕妇病死率为0.00%(0/64);对照组中新生儿死亡3例,病死率为4.69%(3/64)。结论妊娠期高血压疾病合并心力衰竭会增加患者的妊娠风险,增加新生儿病死率;因此,应当给予早期的诊断与必要的阻断治疗。
Objective To study the clinical manifestations of hypertensive disorders complicated with heart failure during pregnancy. Methods One hundred and twenty patients were divided into observation group and control group according to their condition. All the patients in observation group were with hypertensive symptoms of pregnancy complicating heart failure. The control group was single-pregnancy hypertensive symptoms. The average arterial pressure Pressure, albumin, total protein, 24 h urinary protein, HCT, CVT indicators were observed and compared. Results The albumin of the observation group was significantly lower than that of the control group (P <0.05), while the 24 h urine protein was significantly higher than that of the control group (P <0.05). There was no significant difference between the two groups (P> 0.05). Mean mean arterial pressure and HCT in the observation group were significantly higher than those in the control group (P <0.05), while CVT was lower than the control group (P <0.05). In the control group, there were no cases of death of pregnant women, the mortality rate of pregnant women was 0.00% (0/56); in the control group, 4 cases of neonatal death, the mortality rate was 7.14% (4/56). The death rate of pregnant women in the observation group was 0.00% (0/64). There were 3 newborns died in the control group with a mortality rate of 4.69% (3/64). CONCLUSIONS: Hypertensive disorders complicating pregnancy complicating heart failure may increase the risk of pregnancy and increase the neonatal mortality rate. Therefore, early diagnosis and necessary blockade should be given.
