目的:比较可溶性转铁蛋白受体(soluble transferrin receptor,sTfR)、铁蛋白(serum ferritin,SF)和血清铁(serum iron,SI)三种检测指标在诊断妊娠妇女缺铁性贫血中的灵敏度和特异性。方法:分别采用免疫比浊法、免疫化学发光法和fereence法测定50例贫血妊娠妇女和50例健康妊娠妇女的sTfR、SF和SI水平,对实验结果进行比较和分析。结果:50例贫血妊娠妇女的sTfR平均值是3.59±1.66 mg/L,高于临界值(P<0.05),属于贫血范围;SF的平均值10.64±6.18 ng/L,高于临界值,属于正常范围;SI的平均值11.76±7.01μmol/L,高于临界值,属于正常范围。50例正常妊娠妇女的sTfR、SF和SI的平均值分别为2.06±0.35 mg/L(P<0.05)、85.67±45.64n g/L、18.16±5.92μmol/L,三者均属于正常范围内。贫血妊娠妇女的sTfR水平明显高于正常妊娠妇女的sTfR水平(P<0.05),SF和SI水平明显低于正常妊娠妇女水平(P<0.05)。sTfR、SF和SI测定妊娠妇女缺铁的灵敏度分别为74%、46%和44%,特异性分别为86%、96%和94%。结论:sTfR在诊断妊娠妇女缺铁性贫血上灵敏度大于SF和SI,特异性达86%,是早期诊断妊妇缺铁性贫血的良好指标。
OBJECTIVE: To compare the sensitivities of sTfR, SF and serum iron (SI) in the diagnosis of iron deficiency anemia in pregnant women and Specificity. Methods: The levels of sTfR, SF and SI in 50 anemic pregnant women and 50 healthy pregnant women were determined by immunoturbidimetry, immunochemical chemiluminescence and fereence method. The experimental results were compared and analyzed. Results: The mean sTfR of 50 pregnant women with anemia was 3.59 ± 1.66 mg / L, which was higher than the critical value (P <0.05), which belonged to anemia range. The average value of SF was 10.64 ± 6.18 ng / L, which was higher than the critical value Normal range; SI average 11.76 ± 7.01μmol / L, higher than the critical value, belong to the normal range. The mean values of sTfR, SF and SI in 50 normal pregnant women were 2.06 ± 0.35 mg / L (P <0.05), 85.67 ± 45.64ng / L and 18.16 ± 5.92μmol / L respectively, all within the normal range. The level of sTfR in anemic pregnant women was significantly higher than that in normal pregnant women (P <0.05), and the levels of SF and SI in pregnant women were significantly lower than those in normal pregnant women (P <0.05). The sensitivity of sTfR, SF, and SI in determining iron deficiency in pregnant women was 74%, 46%, and 44%, respectively, with a specificity of 86%, 96%, and 94%, respectively. Conclusion: The sensitivity of sTfR in diagnosing iron deficiency anemia in pregnant women is more than that of SF and SI, with a specificity of 86%. It is a good indicator of early diagnosis of iron deficiency anemia in pregnant women.
