目的 探讨鼻咽旁区插植近距离放射治疗技术在鼻咽癌治疗中的作用。方法 初治鼻咽癌患者体外照射 5 6~ 70Gy(T2 期 :5 6~ 6 0Gy ,T3期 :6 6~ 70Gy)后复查CT或MRI发现鼻咽旁区残留病例 6 7例 ,以腔内 +鼻咽旁区插植放射治疗增量 ,咽旁区在模拟机引导下插入 1~ 4根施源管 ,采用荷兰核通公司生产的192 Ir高剂量率近距离治疗机 ,放射剂量 2 .5~ 4.0Gy/次 ,2次 /d ,间隔 >6h ,总剂量 12~ 2 0Gy ,3~ 4d完成。插植组与同期的类似病例 (6 7例 )配对进行研究。结果 鼻咽旁区插植组和对照组的 3年生存率分别为 92 .4%,84.5 %(P >0 .0 5 ) ;3年无局部复发生存率分别为 97.0 %、76 .4%(P <0 .0 5 ) ;3年无远地转移生存率分别为 76 .6 %、6 9.1%(P >0 .0 5 )。鼻咽旁区插植组的晚期放射反应如口干、张口困难发生率均低于对照组。结论 鼻咽旁区插植放射治疗可以显著提高鼻咽癌咽旁区残留的局部控制率 ,拓宽了鼻咽癌近距离放射治疗的适应证 ,是一种有效的补充治疗手段
Objective To investigate the effect of nasopharyngeal parathyroidectomy brachytherapy on the treatment of nasopharyngeal carcinoma. Methods A total of 67 patients with nasopharyngeal carcinoma were retrospectively examined by CT or MRI after in vitro radiotherapy of patients with nasopharyngeal carcinoma (ESR) of 56 ~ 70 Gy (T2 stage: 56 ~ 60 Gy, T3 stage: 66 ~ 70 Gy) + Nasopharyngeal region implanted with radiotherapy increment, the parapharyngeal region in the simulator-guided insertion of 1-4 source tube, using the Netherlands Nuclear Corporation’s 192 Ir high dose rate brachytherapy machine, the dose of 2. 5 ~ 4.0Gy / time, 2 times / d, interval> 6h, the total dose of 12 ~ 20Gy, 3 ~ 4d completed. The implantation group was matched with similar cases (67 cases) in the same period. Results The 3-year survival rates of nasopharyngeal paraplanted group and control group were 92.4% and 84.5% (P> 0.05) respectively. The 3-year local recurrence-free survival rates were 97.0% and 76.4% (P <0.05). The 3-year distant metastasis-free survival rates were 76.6% and 6.11%, respectively (P> 0.05). Late nasopharyngeal implants group of late radiation reactions such as dry mouth, mouth opening difficulties were lower than the control group. Conclusions Radiosurgery of the nasopharyngeal region can significantly improve the local control rate of the parapharyngeal region of nasopharyngeal carcinoma and broaden the indications of brachytherapy for nasopharyngeal carcinoma, which is an effective supplementary treatment
