笔者用RM—3000型多道生理记录仪,同步记录了120例冠心病患者的心阻抗血流图(含微分图)、颈动脉搏动图、心音图、心电图Ⅱ导联、心尖搏动图、桡动脉脉搏图及其微分图;分析了冠心病患者的心功状况。其结果,120例冠心病患者心脏的舒缩功能存在着明显损伤,表现为心缩力指数(HI)、心脏指数(CI)降低、射血前期(PEP)时间延长、左室射血时间(LVET)缩短,PEP/LVET 增大、动脉顺应性(AC),左室顺应性(A/D 值减小)降低,总外周阻力(TPR)增高(P<0.01);这是病理性弦脉产生的病理生理学基础;120例中,有113例表现为弦脉,其桡动脉搏主要表现为弦脉的脉图特征,即脉图收缩波变钝,下降支下降速度非常缓慢,甚至不降反而升高,致使潮波显著或非常显著、主波幅降低、快速射血期及收缩期时值缩短,形成平宽型、园突型或后突型等特征图形,此脉搏图形正是心功减损、动脉顺应性降低、总外周阻力增高、心脏后负荷加大的一种外在表现。从桡动脉脉图和心阻抗血流图等来检测心血管功能,存在较好的相关性,结果趋于一致。
