目的:观察颈动脉狭窄对原发性青光眼视神经纤维层厚度和视野损害的影响。方法:回顾性分析以新郑市人民医院门诊2015年9月至2017年10月原发性青光眼30例(30眼)为观察组,并以同期健康体检的眼部正常者30例(30眼)为对照组。对比两组颈动脉狭窄度、视网膜神经纤维层厚度和视野损害程度。结果:对照组颈动脉无狭窄或轻度狭窄者29例(96.67%),中度狭窄者1例(3.33%),观察组颈动脉中度狭窄者16例(53.33%),严重狭窄者10例(33.33%),极严重狭窄者4例(13.33%),两组血管狭窄程度差异有统计学意义(n Z=7.040,n P=0.000);观察组患眼各象限视网膜神经纤维层厚度均低于对照组(均n P=0.000)。观察组视力(logMAR)明显低于对照组(n P=0.000),模式标准差和全视野青光眼干预研究评分均明显高于对照组(n P=0.000)。观察组颈动脉狭窄程度与视网膜神经纤维层厚度呈负相关(n r=-0.615,n P=0.008),与平均视力、模式标准差和全视野青光眼干预研究评分均呈正相关(n r=0.626、0.593、0.714,n P=0.005,0.014,0.000)。n 结论:颈动脉狭窄程度可对原发性青光眼患者视网膜神经纤维层厚度及视野损害产生显著影响,其病理机制可能存在密切联系。“,”Objective:To observe the influence of carotid artery stenosis on retinal nerve fiber layer thickness and visual field damage in primary glaucoma.Methods:This was a retrospective study.Total of 30 eyes of 30 patients in People’s Hospital of Xinzheng from Sep.2015 to Oct.2017 diagnosed as primary glaucoma were collected as the observation group, and 30 eyes of 30 normal cases received healthy physical examination were collected as the control group.The carotid stenosis severity, retinal nerve fiber layer thickness and visual field damage were compared between the two groups.Results:There were 29 cases (96.67%) without carotid artery stenosis or with mild stenosis and 1 case (3.33%) of moderate carotid artery stenosis in the control group.And there were 16 cases (53.33%) of moderate carotid artery stenosis, 10 cases (33.33%) of severe stenosis and 4 cases (13.33%) of extremely severe stenosis in the observation group.The difference in the vascular stenosis severity was statistically significant between the two groups (n Z=7.040, n P=0.000). The retinal nerve fiber layer thickness in different quadrants in the observation group were significantly lower than those in the control group (n P=0.000). The visual acuity (logMAR) in the observation group was significantly lower than that in the control group (n P=0.000), and the pattern standard deviation and full-field advanced glaucoma intervention study score in the observation group were significantly higher than those in the control group (n P=0.000). The carotid stenosis severity was significantly negatively correlated with retinal nerve fiber layer thickness in the observation group (n r=-0.615, n P<0.05), and it was significantly positively correlated with mean visual acuity, pattern standard deviation and full-field advanced glaucoma intervention study score (n r=0.626, 0.593, 0.714, all n P<0.05).n Conclusion:The carotid stenosis severity can significantly affect the retinal nerve fiber layer thickness and cause visual field damage in patients with primary glaucoma. And the pathological mechanism may be closely related.
