目的探讨小野寺预后营养指数(以下简称小野寺指数)在老年食管癌放射治疗中的临床应用价值。方法选取山东大学齐鲁医院放疗科2011年1月~2014年6月单纯放射治疗的老年食管鳞癌患者93例,根据患者入院后首次血常规和生化检查结果,计算小野寺指数[小野寺指数=白蛋白值(g/L)+5×淋巴细胞总数(×109/L)],并根据小野寺指数将患者分为营养良好组(小野寺指数≥45)和营养不良组(小野寺指数<45),比较两组患者的临床病理特征、生活质量、放射性食管炎分级和肿瘤退缩速度。结果小野寺指数与老年食管癌患者的肿瘤大小(P=0.002)、远处转移(P=0.024)密切相关;生活质量,尤其是治疗态度(P=0.036)、进食量(P=0.043)、日常生活自理能力(P=0.028)方面,营养良好组明显优于营养不良组;营养不良组放射性食管炎的分级显著高于营养良好组(P=0.032);两组肿瘤退缩速度的差异性尚未达到统计学意义(P>0.05)。结论小野寺指数能较好地反映老年食管癌患者放疗前的营养状态、临床病理特征及放疗并发症发生的情况,是一个较有价值的食管癌放疗评估指标。
Objective To investigate the clinical value of Onoji prognostic nutritional index (hereinafter referred to as Onoji index) in radiotherapy of elderly esophageal cancer. Methods Ninety-three elderly patients with esophageal squamous cell carcinoma undergoing radiotherapy alone from January 2011 to June 2014 were enrolled in this study. According to the results of the first routine blood test and biochemical examination after admission, the Onomizu index [Onomizu index = albumin (G / L) + 5 × total number of lymphocytes (× 109 / L)]. The patients were divided into well-nourished group (Onomiji index ≥45) and malnutrition group (Onomiji index <45) Group of patients with clinicopathological features, quality of life, radiation esophagitis grade and tumor regression rate. Results Onion index was closely related to tumor size (P = 0.002) and distant metastasis (P = 0.024) in elderly patients with esophageal cancer. Quality of life, especially treatment attitude (P = 0.036), food intake (P = 0.028), nutrition group was better than malnutrition group. Malnutrition group radiation esophagitis grade was significantly higher than that of well-nutrition group (P = 0.032); the difference of tumor shrinkage rate between the two groups has not yet reached Statistical significance (P> 0.05). Conclusion Onodera index can better reflect the nutritional status, clinicopathological features and radiotherapy complications of elderly patients with esophageal cancer before radiotherapy, and is a valuable evaluation index for radiotherapy of esophageal cancer.
