目的通过使用流式细胞术对子宫内膜异位症(EM)患者外周血单个核细胞(PBMC)中CD4+ CD25+叉头蛋白3(Foxp3+)调节性T细胞(Treg)所占比例的检测,探讨EM与Treg的关系。方法分析2012年8月~2014年4月在该院接受治疗的EM患者(观察组)的临床资料。选择同期经门诊体检无EM的健康的非孕期、非月经期的女性志愿者作为对照组。比较两组受试者以及EM患者不同分期所对应的外周血CD4+CD25+Foxp3+Treg绝对数和在CD4+T淋巴细胞所占比例。结果该研究共纳入受试者78例,其中观察组患者45例,对照组33例。观察组患者的CD4+CD25+Foxp3+、Treg的绝对数量和所占CD4+T淋巴细胞比例均显著高于对照组,差异有统计学意义(t=23.04,P<0.01;t=11.38,P<0.01);观察组患者中Ⅲ~Ⅳ期患者的CD4+CD25+Foxp3+Treg的绝对数量和所占CD4+T淋巴细胞比例均显著高于Ⅰ~Ⅱ期患者,差异有统计学意义(t=17.37,P<0.01;t=18.72,P<0.01)。结论在EM的发生、发展中CD4+CD25+Foxp3+Treg的免疫负性调节作用扮演重要角色。EM形成过程中CD4+CD25+Foxp3+Treg数量的增加可能是一个重要的因素,其表达水平对EM患者预后、病程和病情的判定在临床上有一定的价值意义。
Objective To explore the proportion of CD4 + CD25 + Treg (Treg) in peripheral blood mononuclear cells (PBMCs) of patients with endometriosis (EM) by flow cytometry Relationship between EM and Treg. Methods Clinical data of EM patients treated in this hospital from August 2012 to April 2014 (observation group) were analyzed. Select the same period by the outpatient physical examination without EM of healthy non-pregnancy, non-menstrual women volunteers as a control group. The absolute numbers of CD4 + CD25 + Foxp3 + Tregs and the proportion of CD4 + T lymphocytes in peripheral blood of two groups of subjects and EM patients at different stages were compared. Results A total of 78 subjects were enrolled in this study, including 45 cases in the observation group and 33 cases in the control group. The absolute numbers of CD4 + CD25 + Foxp3 +, Treg and the proportion of CD4 + T lymphocytes in the observation group were significantly higher than those in the control group (t = 23.04, P <0.01; t = 11.38, P < 0.01). The absolute numbers of CD4 + CD25 + Foxp3 + Tregs and the proportion of CD4 + T lymphocytes in stage Ⅲ ~ Ⅳ patients in observation group were significantly higher than those in stage Ⅰ ~ Ⅱ patients, the difference was statistically significant (t = 17.37, P <0.01; t = 18.72, P <0.01). Conclusion The negative immunomodulatory effects of CD4 + CD25 + Foxp3 + Tregs play an important role in the development and progression of EM. The increase of CD4 + CD25 + Foxp3 + Tregs may be an important factor in the process of EM formation. The expression level of EMs in EM patients has a certain clinical value in the prognosis, disease course and disease judgment.
