目的分析濮阳市人间布鲁氏菌病疫情特点,提出防控措施。方法采用描述流行病学方法对病例的“三间分布”及感染来源和途径等进行分析。结果 2006~2014年全市共报告布病病例128例,年报告发病率为0.08/10万~1.48/10万,呈逐年上升趋势。全年均可发病,3~8月份居多,为85例(66.41%);4、5月份达高峰,为37例(28.91%)。各县(区)均有病例报告,集中分布在台前县(64例,50.00%)和濮阳县(21例,16.41%)。患者中男103例,女25例,性别比为4.12∶1;31~60岁95例,占74.22%;职业以农民为主,为116例,占90.63%。调查2013、2014年的76例病例,症状以发热、乏力、多汗和骨关节、肌肉疼痛为主,均有家畜(以羊为主)和/或畜产品接触史,38例(50.00%)患者因饲养放牧、接生幼崽感染,主要分布在濮阳县、南乐县和范县;31例(40.79%)患者因收购贩运活畜、皮毛、羽绒和屠宰、剥死羔皮感染,主要分布在台前县;7例(9.21%)患者因在羊圈玩耍、食用涮羊肉等途径感染,主要分布在濮阳县。结论濮阳市布病疫情呈现发病率升高、流行范围扩大、感染途径多样化的特点。 Objective To analyze the epidemic characteristics of human brucellosis in Puyang City and put forward the prevention and control measures. Methods Descriptive epidemiological methods were used to analyze the “three distributions” of cases and the sources and routes of infection. Results A total of 128 cases of brucellosis were reported in the city from 2006 to 2014, and the annual incidence rate was 0.08 / 100 ~ 1.48 / 100 000, showing an upward trend year by year. The disease can occur throughout the year, mostly in March-August, 85 cases (66.41%); peaked in April and May, 37 cases (28.91%). Each county (district) has case reports, concentrated in Taitian County (64 cases, 50.00%) and Puyang County (21 cases, 16.41%). There were 103 males and 25 females, with a sex ratio of 4.12:1; 95 cases of 31 to 60 years old, accounting for 74.22%; 116 were mainly employed as peasants, accounting for 90.63%. A total of 76 cases were investigated in 2013 and 2014. The symptoms were fever, fatigue, hyperhidrosis, bone and joint and muscle pain. The history of exposure to livestock (mainly sheep) and / or animal products were 38 cases (50.00%), The patients were infected by feeding grazing and receiving pups, mainly in Puyang County, Nanle County and Fanxian County. Among the 31 patients (40.79%), they were mainly infected by the purchase of livestock, fur, feathering and slaughtering, Taichung County; 7 cases (9.21%) were infected by sheepskin and lambs, and were mainly distributed in Puyang County. Conclusion The epidemic situation of brucellosis in Puyang City shows the characteristics of increased incidence, widened epidemic and diversified routes of infection.
