目的探讨子宫内膜癌(EC)患者血浆中CA125与溶血磷脂酸(LPA)的表达及临床意义。方法选取在医院住院治疗的71例EC患者为EC组,同期住院治疗的46例子宫内膜非典型性增生(EAH)患者为EAH组,同期在医院接受健康体检的46例健康女性为对照组,比较3组妇女的基线资料以及血浆LPA、CA125的表达水平。分析EC临床特征与血浆LPA、CA125的关系。结果 3组女性基线资料的差异无统计学意义(P>0.05),EC组患者的LPA与CA125水平及其阳性率均显著高于EAH组和对照组,差异有统计学意义(P<0.01)。EC患者血浆LPA的表达水平与其肿瘤大小关系不明显(P>0.05),与是否为子宫内膜样腺癌、组织学病理分级、TNM分期、淋巴结是否发生转移以及肌层浸润深度相关(P<0.05);血浆CA125的表达水平与是否为子宫内膜样腺癌无关(P>0.05),但与肿瘤大小、组织学病理分级、TNM分期、淋巴结是否发生转移以及肌层浸润深度相关(P<0.05)。结论在EC的鉴别诊断及早期诊断方面,血浆LPA的临床应用价值优于CA125;在EC预后判断方面,LPA与CA125均是重要指标。
Objective To investigate the expression and clinical significance of plasma CA125 and lysophosphatidic acid (LPA) in patients with endometrial carcinoma (EC). Methods Seventy-one patients with EC who were hospitalized in the hospital were selected as EC group. 46 patients with endometrial atypical hyperplasia (EAH) who were hospitalized in the same period were selected as EAH group and 46 healthy women undergoing physical examination during the same period as control group The baseline data of three groups of women and the plasma levels of LPA and CA125 were compared. Analysis of the clinical features of EC and plasma LPA, CA125 relationship. Results There was no significant difference in baseline data between the three groups (P> 0.05). The levels of LPA and CA125 and their positive rates in EC group were significantly higher than those in EAH group and control group (P <0.01) . There was no significant correlation between the expression of LPA and the size of tumor in patients with EC (P> 0.05), and whether it was endometrial adenocarcinoma, histological grade, TNM stage, lymph node metastasis and depth of myometrial invasion (P < 0.05). The expression level of plasma CA125 was not correlated with endometrial adenocarcinoma (P> 0.05), but correlated with tumor size, histological grade, TNM stage, lymph node metastasis and depth of myometrial invasion (P < 0.05). Conclusion The clinical value of plasma LPA is better than that of CA125 in the differential diagnosis and early diagnosis of EC. LPA and CA125 are both important indexes in judging EC prognosis.
