目的研究腺瘤型原发性醛固酮增多症APA高醛固酮血症对心脏的损害,探讨临床治疗对策。方法收集156例APA和96例原发性高血压(EH)患者高血压病程、血浆醛固酮浓度、血钾浓度、肾素活性、心电图及超声心动图资料,分析心脏损害与各指标的相关性。结果APA组中150例行心电图和超声心动图检查,其中54例存在心电图异常,26例存在心动超声异常。而EH组96例中心电图异常12例,心动超声异常7例。两组比较有显著性差异。APA患者的心脏损害较为多见。结论高醛固酮血症是引起腺瘤型醛固酮增多症患者心脏损害的主要因素之一,应该引起临床重视。
Objective To study the damage to the heart caused by APA hyperaldosteronism in patients with adenoid primary hyperaldosteronism and to explore the clinical treatment strategies. Methods The course of hypertension, plasma aldosterone concentration, serum potassium concentration, renin activity, electrocardiogram (ECG) and echocardiography data of 156 patients with APA and 96 patients with essential hypertension (EH) were collected. The correlation between heart damage and each index was analyzed. Results In the APA group, 150 patients underwent electrocardiogram and echocardiography. Among them, 54 patients had abnormal electrocardiogram and 26 patients had abnormal echocardiogram. EH group, 96 cases of abnormal electrocardiogram in 12 cases, abnormal echocardiography in 7 cases. There were significant differences between the two groups. APA patients with heart damage is more common. Conclusion Hyperaldosteronism is one of the major causes of cardiac damage in patients with adenoma of aldosteronism, which should be clinically valued.
