目的了解基质辅助激光解吸电离-飞行时间质谱(Matrix-Assisted Laser Desorption Ionization-Time Of Flight Mass Spectrometry,MALDI-TOF MS)在产碳青霉烯酶的鲍曼/醋酸钙不动杆菌复合体快速检测中的临床应用,为临床治疗提供参考。方法收集2015年1月-2016年5月从四川大学华西医院住院患者标本中分离的鲍曼/醋酸钙不动杆菌复合体48株,采用Vitek 2 Compact自动化系统和纸片扩散法检测菌株对亚胺培南的体外敏感性;使用多重聚合酶链反应检测菌株携带碳青霉烯酶基因的情况;运用MALDI-TOF MS检测产碳青霉烯酶的菌株。结果 48株试验菌株包含26株亚胺培南耐药株,22株亚胺培南敏感株;26株耐药株均携带有ISAba1-bla OXA-23-like碳青霉烯酶基因,均能被MALDI-TOF MS检测为产碳青霉烯酶的菌株。结论 MALDI-TOF MS可以快速检测产碳青霉烯酶的鲍曼/醋酸钙不动杆菌复合体,为该类病原菌引起感染的早期治疗提供参考。
Objective To investigate the rapid detection of carbopenem-resistant Acinetobacter baumannii complex by Matrix-Assisted Laser Desorption Ionization-Time Of Flight Mass Spectrometry (MALDI-TOF MS) In the clinical application, provide a reference for clinical treatment. Methods Forty eight strains of Acinetobacter baumannii / Acinetobacter baumannii isolated from inpatients in West China Hospital of Sichuan University from January 2015 to May 2016 were collected. Vitek 2 Compact automated system and disk diffusion method were used to detect the isolates of Acinetobacter baumanii In vitro sensitivity of amipenem; detection of strains carrying carbapenemase by multiplex polymerase chain reaction; detection of carbapenemase producing strains by MALDI-TOF MS. Results The 48 strains of test strains contained 26 strains of imipenem and 22 strains of imipenem. The resistant strains carrying ISAba1-bla OXA-23-like carbapenemase were all resistant Carbapenemase-producing strains were detected by MALDI-TOF MS. Conclusion MALDI-TOF MS can rapidly detect the carbamazepine-producing Acinetobacter baumannii complex and provide a reference for the early treatment of this pathogen-causing infection.
