目的:通过三维适形放疗联合化疗粒子植入治疗不可切除的恶性实体肿瘤。方法:对住院的15例不可切除的恶性实体肿瘤患者在进行三维适形放疗前1周行化疗粒子植入,植入药物剂量依肿瘤大小而定。术后第7天开始行三维适形放疗,CT定位后传入三维治疗计划系统,勾画靶区,设肿瘤为GTV,GTV外扩1.0~1.5 cm为PTV,每日200 cGy,总量为5 000~6 000 cGy。结果:2例疗效达到CR,10例疗效达到PR,2例疗效为PD,1例疗效为SD。总有效率为80%。结论:三维适形放疗联合化疗粒子植入技术对于不可切除的恶性实体肿瘤是一种有效的治疗手段。
OBJECTIVE: To treat unresectable malignant solid tumors by three-dimensional conformal radiotherapy combined with chemotherapy and particle implantation. Methods: Fifteen patients with unresectable malignant solid tumors in hospital were treated with chemotherapeutic particles one week before three-dimensional conformal radiotherapy, and the dose of the implanted drug was determined according to the size of the tumor. Three-dimensional conformal radiotherapy was started on the 7th day after operation. After CT localization, it was introduced into the three-dimensional treatment planning system and the target area was delineated. The tumor was GTV, and the PTV was 1.0-1.5 cm outside the GTV. The total daily dose was 200 cGy 000 ~ 6 000 cGy. Results: Two cases achieved CR, 10 cases achieved PR, two cases treated with PD, and one case treated with SD. The total effective rate is 80%. Conclusion: Three-dimensional conformal radiotherapy combined with chemotherapy and particle implantation is an effective treatment for unresectable malignant solid tumors.
