为了比较锥状点式接触接骨板与普通接骨板的力学性能及对局部皮质骨微循环的影响,采用材料、尺寸完全一致的模拟骨,分别用两种接骨板固定,进行三点弯曲实验和扭转实验比较二者的力学性能。另用14只山羊,4只正常对照,10只行胫骨中段横行截骨后分别用两种接骨板固定。术后3天、2周、4周和9周处死动物,用放射性微球局部血流量测定法评价接骨板下和对侧皮质骨血流量。结果发现:两种接骨板的抗弯能力无显著差异;当转角大于18时,普通接骨板的抗扭能力要高于锥状点式接触接骨板。普通接骨板板下皮质骨持续缺血,而锥状点式接触接骨板板下皮质骨血循环明显好于普通接骨板组,术后2周、4周、9周时分别是后者的2.5倍、9.9倍和2.8倍。接骨板对侧皮质骨缺血是短暂的,术后2周就已恢复。因此,尽管锥状点式接触接骨板的抗扭能力稍低于普通接骨板,但它在保护接骨板下血运方面明显优于普通接骨板,因而更有利于骨折愈合。
In order to compare the mechanical properties of the tapered point contact plate and the ordinary plate and the effect on the local cortical bone microcirculation, the simulated bone with the same material and size was used to fix the bone with two kinds of plates. Torsional experiments compare the mechanical properties of the two. In addition, 14 goats and 4 normal controls were used. 10 of them were treated with transverse osteotomy and then fixed with two kinds of plates. Animals were sacrificed at 3, 2, 4, and 9 weeks after surgery. The blood flow under the subplate and contralateral cortical bone was evaluated using a radioimmunosphere local blood flow assay. The results showed that there was no significant difference in the bending capacity of the two types of bone plates. When the angle of rotation was greater than 18, the torsional resistance of ordinary plates was higher than that of tapered point contact plates. The cortical bone under the common bone plate sustained ischemia, and the blood circulation of cortical bone under the conical point contact plate was significantly better than that of the normal plate group. The latter was at 2 weeks, 4 weeks, and 9 weeks after operation. 5 times, 9.9 times, and 2.8 times. The contralateral cortical bone ischemia of the plate was transient and recovered 2 weeks after surgery. Therefore, although the anti-twisting ability of the tapered point contact plate is slightly lower than that of the ordinary plate, it is superior to the normal plate in protecting the blood supply under the plate, and is therefore more conducive to fracture healing.
