血管转换酶抑制剂是一类重要的抗高血压药物,能引起皮疹、血管神经性水肿等皮肤不良反应,且没有剂量相关性。皮疹病人的皮肤活检标本组织学表现主要为表皮淋巴细胞、组织细胞浸润,毛细血管内皮肿胀。免疫细胞化学证实为T细胞。血管神经性水肿罕见,后果较严重,而皮疹常见,反应较轻。停用该类药物后上述反应消失。皮肤不良反应的发生机制尚未明确,除特异质外,可能主要与这类药物非特异性抑制激肽酶活性,导致缓激肽水平提高等因素有关。
Angiotensin-converting enzyme inhibitors are a class of important anti-hypertensive drugs that cause skin side effects such as rashes and angioneurotic edema with no dose-related effects. Skin biopsy specimens of patients with rash mainly histological manifestations of epidermal lymphocytes, tissue cell infiltration, capillary endothelial swelling. Immunocytochemistry confirmed T cells. Vascular edema is rare, the consequences are more serious, while the rash is common, less reaction. The withdrawal of these drugs after the reaction disappeared. The mechanism of skin adverse reactions is not yet clear, in addition to specific, may be mainly non-specific inhibition of these drugs kininase activity, resulting in increased levels of bradykinin and other factors.
