目的 探讨V型斜视理想的手术方法。方法 回顾性分析 1 999～2 0 0 2年由作者施行的斜视手术中 ,V型斜视术后随访 3～ 6月者 34例。无斜肌功能过强的行水平肌后徙或 /和缩短的同时垂直移位 ,伴下斜肌功能亢进的除以上术式外再加下斜肌减弱术。结果 术后随访正位 32例 ,欠矫 2例 ,V征 (- ) 31例 ,V征 (+) 3例。同视机检查 :双眼视功能Ⅲ级 2 3例 ,Ⅱ级 2例 ,Ⅰ级 6例 ,不理解 3例。结论 水平肌垂直移位术矫正V型斜视具有满意疗效 ,其中伴下斜肌亢进的病例 ,还需减弱下斜肌。另外 ,矫正V征对于双眼单视功能的建立极为有利 ,术后短期内不经功能训练即可使多数患者的异常网膜对应消失而恢复正常双眼视觉
Objective To investigate the ideal surgical method of V-type strabismus. Methods Retrospective analysis of strabismus surgery performed by the author from 1999 to 2002, 34 cases were followed up for 3 to 6 months after V-shaped strabismus. Without oblique muscle function of the line horizontal muscle posterior migration or / and shortening of the vertical shift at the same time, with the next oblique muscle function hyperthyroidism in addition to the above surgery plus oblique muscle weakening. Results 32 cases were followed up, 2 cases were undercorrected, 31 cases were V sign (-), 3 cases were V sign (+). With the same examination: binocular visual function class Ⅲ 23 cases, Ⅱ class in 2 cases, Ⅰ class in 6 cases, do not understand the 3 cases. Conclusion The level of vertical muscle shift surgery to correct V-shaped strabismus has a satisfactory curative effect, which accompanied by hypoechoic cases, also need to weaken the lower oblique. In addition, the correction of V signs for the establishment of binocular monophasic function is extremely beneficial, short-term postoperative without functional training can make the majority of patients corresponding to the disappearance of abnormal omentum and restore normal binocular vision
