目的:分析脐动脉血气分析与新生儿窒息的临床相关性,探讨脐动脉血气分析在新生儿窒息诊断中的应用价值。方法:选择2010年7月~2012年9月,在我院分娩的单胎足月新生儿150例,以上入选新生儿均进行脐动脉血气分析与出生后1min Apgar评分,分析新生儿脐动脉血气分析结果与出生后1min Apgar评分的相关性,并探讨其在新生儿窒息诊断中的应用价值。结果:新生儿脐动脉血PO2、pH值、BE值与新生儿Apgar评分,呈正相关性(P<0.05);脐动脉血PO2与新生儿Apgar评分,呈负相关性(P<0.05);重度窒息组与轻度窒息组和正常组的脐动脉血气分析结果,差异均具有(高度)统计学意义(P<0.05,P<0.01);重度窒息组分别与轻度窒息组和正常组的pH值<7.20与pH值>7.25患儿例数比较,差异均具有(高度)统计学意义(P<0.05,P<0.01)。结论:在新生儿窒息的临床诊断中,脐动脉血气分析能够弥补传统Apgar评分的不足,对新生儿窒息的早期诊断与预后判断具有重要的临床意义。
Objective: To analyze the clinical correlation between umbilical arterial blood gas analysis and neonatal asphyxia and to explore the value of umbilical artery blood gas analysis in the diagnosis of neonatal asphyxia. Methods: From July 2010 to September 2012, 150 singletonic full-term newborns were delivered in our hospital. All the above neonates were tested for umbilical arterial blood gas analysis and Apgar score at 1 minute after birth to analyze neonatal umbilical arterial blood gas The correlation between the results and Apgar score at 1 minute after birth was analyzed and its value in the diagnosis of neonatal asphyxia was explored. Results: There was a positive correlation between PO2, pH value and BE value in neonatal umbilical artery and neonatal Apgar score (P <0.05); PO2 in umbilical arterial blood was negatively correlated with neonatal Apgar score (P <0.05) The results of umbilical arterial blood gas analysis in asphyxia group, mild asphyxia group and normal group were all statistically significant (height) (P <0.05, P <0.01); severe asphyxia group had no significant difference with mild asphyxia group and normal group The difference was significant (height) (P <0.05, P <0.01) when the value of <7.20 was compared with the number of children with pH> 7.25. Conclusion: In the clinical diagnosis of neonatal asphyxia, umbilical artery blood gas analysis can make up for the lack of the traditional Apgar score, which has important clinical significance for the early diagnosis and prognosis of neonatal asphyxia.
