目的讨论新生儿缺血缺氧性脑病(hypoxic ischemic encephalopathy,HIE)外周血T淋巴细胞亚群与免疫球蛋白的表达及相关性,综合分析体液免疫和细胞免疫在新生儿缺血缺氧性脑病中的变化特点。方法分别选择不同程度的HIE患儿29例作为HIE组和同期出生的正常新生儿20例作为正常对照组,采用流式细胞法测定外周血中T淋巴细胞亚群,免疫比浊法测定免疫球蛋白,SPSS17.0统计分析软件分析结果。结果 T淋巴细胞亚群比较:HIE组CD3+、CD4+较正常组明显减低,差异有统计学意义(P<0.05);HIE轻度组与中重度组比较无统计学意义(P>0.05),CD8+、CD4+/CD8+在正常对照组、HIE轻度组和HIE中重度组三组之间的表达差异无统计学意义(P>0.05)。免疫球蛋白的比较:HIE组IgM较正常对照组低,差异有统计学意义(P<0.05),轻度组与中重度组比较差异无统计学意义(P>0.05)。IgA、IgG在正常对照组、HIE轻度组和HIE中重度组三组之间的表达差异无统计学意义(P>0.05)。结论新生儿HIE对细胞免疫系统的影响主要表现是CD3+和CD4+将低;而对体液免疫系统的影响主要表现是免疫球蛋白IgM降低。
Objective To discuss the expression and correlation of T lymphocyte subsets and immunoglobulins in peripheral blood of neonates with hypoxic ischemic encephalopathy (HIE). To analyze the relationship between humoral immunity and cellular immunity in neonatal hypoxic-ischemic encephalopathy In the changes in the characteristics. Methods Twenty-nine children with HIE were selected as the HIE group and 20 normal newborn infants of the same period as the normal control group. The T lymphocyte subsets in the peripheral blood were measured by flow cytometry and the immunoglobulin Protein, SPSS17.0 statistical analysis software analysis results. Results Compared with normal group, CD3 + and CD4 + levels in HIE group were significantly lower than those in normal group (P <0.05). There was no significant difference between mild HIE group and moderate or severe HIE group (P> 0.05) There was no significant difference in the expression of CD4 + / CD8 + between the normal control group, mild HIE group and moderate and severe HIE group (P> 0.05). Immunoglobulin comparison: HIE group IgM lower than the normal control group, the difference was statistically significant (P <0.05), mild and moderate severe group was no significant difference (P> 0.05). There was no significant difference in the expression of IgA and IgG between the normal control group, mild HIE group and moderate and severe HIE group (P> 0.05). Conclusion The main effect of neonatal HIE on the cellular immune system is that CD3 + and CD4 + will be low. The impact on the humoral immune system is mainly due to the decrease of immunoglobulin IgM.
